尿常规与尿微量白蛋白检测在糖尿病早期肾损伤临床诊断中的价值分析

Analysis of the Value of Urine Routine and Urine Microalbumin Testing in the Clinical Diagnosis of Early Diabetic Kidney Injury

目的 探究对糖尿病早期肾损伤患者实施尿常规与尿微量白蛋白检测的价值。方法 选取2019年1月—2021年12月期间尤溪县总医院收治的120例疑似存在早期肾损伤的糖尿病患者作为研究对象,依次为患者进行尿常规以及尿微量白蛋白检测,对比两种检查的检出率;再将肾脏活检作为糖尿病早期肾损伤的“金标准”,计算比较两种检查的灵敏度、特异度。结果 尿微量白蛋白检测的检出率(90.83%)高于尿常规检测(45.83%),差异有统计学意义(χ^(2)=56.149,P<0.001);尿常规检测的灵敏度为45.95%,低于尿微量白蛋白检测的灵敏度97.30%,差异有统计学意义(P<0.05);尿常规检测的特异度为55.56%,与尿微量白蛋白检测的特异度88.89%对比,差异无统计学意义(P>0.05)。结论 尿常规与尿微量蛋白检测对于糖尿病早期肾损伤均有一定的诊断价值,而尿微量蛋白的检测灵敏度更高,能够更好地识别存在早期肾损伤的糖尿病患者,提高早期肾损伤的检出率。

Objective To explore the value of routine urine test and microalbuminuria test in patients with early renal injury of diabetes.Methods 120 diabetes patients suspected of having early renal injury admitted to Youxi County General Hospital from January 2019 to December 2021 were selected as the research objects.Routine urine test and microalbumin test were carried out for the patients in turn,and the detection rates of the two tests were compared;then renal biopsy was taken as the"gold standard"of early renal injury in diabetes,and the sensitivity and specificity of the two examinations were calculated and compared.Results The detection rate of urine microalbumin test(90.83%)was higher than that of routine urine test(45.83%),and the difference was statistically significant(χ^(2)=56.149,P<0.001).The sensitivity of urine routine test was 45.95%,which was lower than that of urine microalbumin test(97.30%),and the difference was statistically significant(P<0.05).The specificity of routine urine test was 55.56%,and there was no statistically significant difference compared with the specificity of urine microalbumin test 88.89%(P>0.05).Conclusion Routine urine test and urinary micro protein test have certain diagnostic value for early renal in⁃jury of diabetes,while urinary micro protein test has higher sensitivity,which can better identify diabetes patients with early renal injury and improve the detection rate of early renal injury.
