冠心病患者脂蛋白相关磷脂酶A2水平与肌酐清除率的相关性分析

Correlation Analysis of Lipoprotein Associated Phospholipase A2 Levels and Endogenous Creatinine Clearance in Patients with Coronary Artery Disease

摘要 目的探讨脂蛋白相关磷脂酶A2(lipoprotein associated phospholipase A2,Lp-PLA2)的水平与肌酐清除率(endogenous creatinine clearance,Ccr)的相关性。方法选取2018年1月—2020年12月期间在徐州医科大学附属市立医院心内科住院的冠心病患者170例为研究对象,根据入院后Lp-PLA2的升高幅度不同,将患者分为显著升高组(Lp-PLA2≥600 ng/mL,42例),中度升高组(400 ng/mL≤LpPLA2<600 ng/mL,63例)及轻度升高组(200≤Lp-PLA2<400 ng/mL,65例)。监测比较3组空腹血糖、肌酐、尿素氮、肌酐清除率等指标水平,分析上述指标与Lp-PLA2的相关性。结果显著升高组的Ccr(75.857±37.276)mL/min与中度升高组(106.080±38.535)mL/min、轻度升高组(102.387±32.994)mL/min对比,差异有统计学意义(P<0.05),中度升高组与轻度升高组对比,差异无统计学意义(P>0.05)。结论Lp-PLA2与Ccr水平呈负相关,可作为间接反映肾功能的直观新型指标。 Objective To investigate the correlation between the level of lipoprotein associated phospholipase A2(Lp-PLA2)and endogenous creatinine clearance(Ccr).Methods 170 patients with coronary heart disease who were hospi⁃talized in the Department of Cardiology of the Affiliated Municipal Hospital of Xuzhou Medical University from Janu⁃ary 2018 to December 2020 were selected as the study subjects.According to the different levels of increase in Lp-PLA2 after admission,the patients were divided into significantly elevated group(Lp-PLA2≥600 ng/mL,42 cases),moderately elevated group(400 ng/mL≤LpPLA2<600 ng/mL,63 cases)and slightly elevated group(200≤Lp-PLA2<400 ng/mL,65 cases).Monitored and compared the levels of fasting plasma glucose,creatinine,urea nitrogen,creati⁃nine clearance rate and other indicators in the three groups,and analyzed the correlation between the above indicators and Lp-PLA2.Results Ccr of the significantly elevated group(75.857±37.276)mL/min,the moderately elevated group(106.080±38.535)mL/min and the mildly elevated group(102.387±32.994)mL/min were compared,the differ⁃ence was statistically significant(P<0.05).There was no statistically significant difference between the moderate el⁃evated group and mildly elevated group(P>0.05).Conclusion Lp-PLA2 is negatively correlated with Ccr level,which can be used as a visual novel indicator of renal function indirectly.
