阿加曲班对伴有早期神经功能恶化的急性缺血性卒中患者临床疗效、神经功能及日常生活能力的影响

Effect of Argatroban on Clinical Efficacy,Neurological Function and Daily Living Ability in Patients with Acute Ischemic Stroke with Early Neuro⁃logical Deterioration

目的探讨伴有早期神经功能恶化的急性缺血性卒中(acute ischemic stroke,AIS)患者采用阿加曲班治疗对临床疗效、日常生活能力、神经功能的影响。方法选取2019年1月—2022年6月江苏省张家港市第一人民医院收治的80例伴有早期神经功能恶化的AIS患者,根据随机摸球法分为观察组与对照组,各40例。对照组采用常规治疗,观察组在对照组基础上采用阿加曲班治疗,比较两组患者的临床疗效、神经功能及日常生活能力。结果观察组的治疗总有效率(95.00%)高于对照组(77.50%),差异有统计学意义(χ^(2)=5.165,P<0.05);治疗后,两组NIHSS、改良Rankin评分均明显降低,且观察组均低于对照组,差异有统计学意义(P<0.05);治疗后,两组FMA、ADL、改良Barthel指数评分均明显提高,且观察组均高于对照组,差异有统计学意义(P<0.05)。结论阿加曲班应用在伴有早期神经功能恶化的AIS患者治疗中,可提高临床疗效,改善神经功能,提高日常生活能力。

Objective To explore the impact of argatroban on clinical efficacy,ability of daily living,and neurological function in patients with acute ischemic stroke(AIS)accompanied by early neurological deterioration.Methods 80 AIS patients with early neurological deterioration admitted to the Zhangjiagang First People's Hospital from January 2019 to June 2022 were selected,according to random ball-touching method,they were divided into observation group and control group,35 cases in each group.The control group was treated with conventional treatment,and the observa⁃tion group was treated with argatroban on the basis of the control group.The clinical efficacy,neurological function and daily living ability of the two groups were compared.Results The total effective rate of observation group(95.00%)was significantly higher than 77.50%of control group,the difference was statistically significant(χ^(2)=5.165,P<0.05).After treatment,the NIHSS and modified Rankin scores of the two groups were significantly lower,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).After treat⁃ment,the scores of FMA,ADL and modified Barthel index in the two groups were significantly improved,and the ob⁃servation group was higher than the control group,and the difference was with statistically significant(P<0.05).Con⁃clusion The application of argatroban in the treatment of AIS patients with early neurological deterioration can im⁃prove clinical efficacy,improve neurological function,and enhance the ability of daily living.
