MRI在先天性多拇畸形WasselⅣ型术前鉴别中的应用

Application of MRI in preoperative differentiation of Wassel typeⅣcongenital thumb polydactyly

目的探讨核磁共振在婴幼儿先天性多拇畸形WasselⅣ型患者术前鉴别中的作用。方法统计我院自2016年1月至2020年12月收治且完成术前X线片及MRI检查的先天性多拇畸形患者266例286指,针对这两种辅助检查进行单独阅片分型,并将结果与术中分型进行比较。结果通过术前X线片阅片分型的WasselⅣ型共123指,与术中分型比对,约18.7%的患者存在分型错误,分型错误最多的是WasselⅢ型。通过核磁共振成像阅片分型的WasselⅣ型患者共99例,无一例出现分型错误。结论先天性多拇畸形Wassel分型中以Ⅳ型最为多见,仅通过术前X线片进行分型存在较高的错误率。核磁共振能有效降低术前分型的错误率,还能获得全面的术前信息,更精确地指导治疗。

Objective To investigate the role of magnetic resonance imaging(MRI)in the preoperative differentiation of Wassel typeⅣcongenital thumb polydactyly in infants.Methods A total of 286 fingers of 266 patients with congenital thumb polydactyly who were admitted to our hospital from January 2016 to December 2020 and completed preoperative X-ray and MRI examinations were statistically analyzed,and the results were compared with the intraoperative classification according to the two auxiliary examinations.Results A total of 123 Wassel typeⅣfingers were classified by preoperative X-ray film reading.Compared with intraoperative classification,about 18.7%of the patients had classification errors,and Wassel typeⅢhad the most classification errors.A total of 99 Wassel typeⅣpatients were classified by MRI film reading,and none of them had classification errors.Conclusion In Wassel classification of congenital thumb polydactyly,typeⅣis the most common,and there is a high error rate in classification only by preoperative X-ray films.MRI can effectively reduce the error rate of preoperative classification,and also obtain comprehensive preoperative information to guide treatment more accurately.
