血液透析患者认知功能管理指标的构建

Construction of management index on cognitive function for hemodialysis patients

目的构建血液透析患者认知功能管理指标,用于指导临床对认知功能障碍患者实施早期干预,实现科学化、规范化的管理。方法通过专家焦点小组讨论、采用德尔菲咨询法对13名相关领域专家进行2轮专家函询,采用层次分析法确定各指标权重。结果2轮专家函询的问卷有效回收率分别为92.31%和100%,专家权威系数均为0.850,肯德尔和谐系数分别为0.203、0.224,差异有统计学意义(P<0.001)。最终构建的血液透析患者认知功能管理指标包括血液透析前认知功能评估预测、血液透析中认知功能干预、血液透析间期认知功能远程预控监控三大模块,即3个一级指标、11个二级指标、22个三级指标。结论构建的血液透析患者认知功能管理指标具有科学性和实用性,可以为开展患者认知功能护理研究及实践提供借鉴。

Objective To construct the management index on cognitive function for hemodialysis patients,guide early intervention on patients with cognitive dysfunction and realize scientific and standard management.Method Perform two rounds of expert consultation on 13 experts in related fields by focus group discuss and Delphi method.Confirm the weight of each index by analytic hierarchy process method.Result The valid recovery rates of questionnaire in two rounds of expert consultation are 92.31%and 100%.The expert authority coefficients are all 0.850.The Kendall coefficient of concordance are 0.203 and 0.224.The difference is significant(P<0.001).The management index on cognitive function for hemodialysis patients finally cunstructed includes three modules of pre-hemodialysis cognitive function evaluation,cognitive function intervention during hemodialysis and remote monitoring on interdialytic cognitive function,namely 3 first class indexes,11 second class indexes and 22 third class indexes.Conclusion The management index on cognitive function for hemodialysis patients cunstructed is scientific and practical,which can provide reference for nursing research and practice on cognitive function.
