慢性肾功能不全患者住院费用影响因素分析

Analysis on the Influencing Factors of Hospitalization Expenses of Patients with Chronic Kidney Disease

目的 分析某院慢性肾功能不全患者的住院费用构成及其影响因素,为精细化控制医疗费用提供参考。方法 回顾性分析2020年1月1日-2021年12月31日某院412例慢性肾功能不全患者病例资料,利用非参数检验、多元线性回归分析对住院费用及其影响因素进行统计分析。结果 纳入研究的412病例,平均住院费用为16632.66±6856.05元,住院费用中位数为15570.10(11955.24-20368.11)元,其中药占比最高(34.99%),其次为治疗费用(25.06%)与诊断费用占比较高(20.22%)。患者住院费用多元回归分析结果显示,影响慢性肾功能不全患者住院费用的显著因素是住院天数、并发症个数及30天是否再入院(F=50.948,P<0.001,R^(2)=0.422)。结论 中西药费占总费用最高,影响住院费用的主要因素是住院天数、并发症个数。合理用药,缩短住院天数,是控制慢性肾功能不全患者住院费用的重要措施。

Objectives To analyze the composition and influencing factors of hospitalization expenses of chronic kidney disease patients in a hospital, and provide reference for refined control of medical cost.Methods The data of 412 chronic kidney disease patients in a hospital from January 1st, 2020 to December 31st, 2021 was retrospectively analyzed, and the hospitalization expenses and its influencing factors were statistically analyzed by non-parametric test and multiple linear regression analysis. Results Among the 412cases included in the study, the average hospitalization cost was 16632.66±6856.05 yuan, and the median hospitalization cost was 15570.10(11955.24~20368.11)yuan. The proportion of medicine expenses was the highest(34.99%), followed by the treatment expenses(25.06%) and diagnosis expenses(20.22%).Multivariate regression analysis showed that the significant factors influencing the hospitalization expenses of patients with chronic renal disease were the length of stay, the number of complications and whether they were re-hospitalized within 30 days(F = 50.948, P<0.001, R^(2)=0.422). Conclusions The proportion of medicine expenses was the highest. Optimizing rational medical use, shortening the length of stay and improving patients’ awareness of disease prevention and treatment were important measures t o control the hospitalization expenses of patients with chronic renal disease.
