Analysis on the Hospitalization Costs of Lung Cancer Surgery Patients and Its Influencing Factors

目的 研究肺癌手术患者住院费用的构成情况及影响因素,为合理控制住院费用、减轻患者经济负担提供参考。方法 以某院2016年1月1日-2020年12月31日期间7523例肺癌手术患者住院费用为研究对象,采用描述性统计方法分析住院费用构成,两组之间的均值比较采用t检验,多组之间的均值比较采用方差分析检验。将单因素分析的有效结果再进行多元逐步回归分析。结果 肺癌手术患者平均住院费用为80 240元,五年间呈下降趋势。主要构成为材料费和药品费,材料费均值38 254.4元(47.68%),药品费均值为19848.75元(24.74%),两项费用占总住院费用的72.42%。多元逐步回归分析结果显示F=725.382,P<0.05.影响住院费用的因素依次为住院天数、出院年份、手术方式、其他诊断个数、年龄、性别。结论 药品和耗材费用的降低是住院费用降低的重要因素。合理控制患者住院天数和加强微创手术费用管理可以有效降低肺癌手术患者住院费用。

Objectives To understand the composition and main influencing factors of hospitalization costs in lung cancer surgery patients, and provide basis for reducing financial burden and reasonably controlling hospitalization costs. Methods To select the lung cancer surgery in a hospital from January 1st, 2016 to December 31st, 2020 as the research objects. Descriptive statistics was used to analyze the composition of hospitalization expenses, t test was used to compare the mean between the two groups and the variance analysis was used to compare the mean between groups. The effective results of single factor analysis were then analyzed by multivariate analysis. Results The average hospitalization costs of lung cancer surgery was 80240.87 yuan. It showed a downward trend in the past five years. The cost of medicinal materials and drugs were the main components of expenses, Average cost of medicinal materials was 38254.4yuan, average cost of drugs was 19848.75 yuan,accounting for 72.42% of the total cost. The multivariate analysis showed that F=725.382, P<0.05. The main factors that influenced the hospitalization costs including hospitalization days,years of hospitalization,the way of operation, number of other diagnoses, age and gender. Conclusions The decrease of drugs and medicinal materials were the important factors in reducing hospitalization expenses. Reasonably optimizing the number of days in hospital and strengthen the cost management of minimally invasive surgery could effectively reduce the hospitalization costs of lung cancer surgery patients.
