3D腹腔镜联合加速康复外科模式在结直肠癌中的应用

Application of 3D laparoscope combined with enhanced recovery after surgery in colorectal cancer

目的探讨3D腹腔镜联合加速康复外科(ERAS)在结直肠癌(CRC)中的应用效果。方法选取我院收治的CRC患者100例,根据随机数字表法将患者分为观察组(采用3D腹腔镜联合ERAS治疗)和对照组(采用3D腹腔镜治疗),每组50例。比较2组患者手术相关指标、术后恢复相关指标及术后并发症发生情况。采用结直肠癌生活质量专用量表(EORTC QLQ-CR38)评价患者术前及术后1个月、3个月、6个月的生活质量。结果2组手术时间、术中出血量比较差异无统计学意义(P>0.05);观察组术后排气时间、经口进食时间、下床活动时间、术后住院时间短于对照组(P<0.05)。观察组术后并发症发生率低于对照组(P<0.05)。2组术前EORTC QLQ-CR38评分比较差异无统计学意义(P>0.05);与术前比较,术后1个月EORTC QLQ-CR38评分均降低,但观察组术后3个月和6个月EORTC QLQ-CR38评分均升高,且观察组EORTC QLQ-CR38评分高于对照组(P<0.05)。结论3D腹腔镜联合ERAS治疗CRC可有效缩短手术时间,加速患者术后康复,降低并发症发生率,有利于提高患者术后生活质量。

Objective To explore the application effect of 3D laparoscope combined with enhanced recovery after surgery(ERAS)in colorectal cancer(CRC).Methods A total of 100 patients with CRC admitted in our hospital were enrolled and divided into the observation group(received 3D laparoscope combined with ERAS)and the control group(received 3D laparoscope)according to random number table method,with 50 cases in each group.The surgical related indexes,postoperative recovery-related indexes and occurrence of postoperative complications between the two groups were compared.The quality of life in patients before surgery and 1 month,3 months and 6 months after surgery was evaluated by EORTC quality of life questionnaires of colorectal cancer-38(EORTC QLQ-CR38).Results There was no statistically significant difference in the operation time or intraoperative blood loss between the two groups(P>0.05).The postoperative exhaust time,oral feeding time,ambulation time and postoperative hospital stay in the observation group were shorter than those in the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).There was no statistically significant difference in the EORTC QLQ-CR38 scores before surgery between the two groups(P>0.05),and the EORTC QLQ-CR38 scores 1 month after surgery in the two groups were decreased(P<0.05),but the EORTC QLQ-CR38 scores 3 months and 6 months after surgery in the observation group were increased compared with that before surgery and that of the control group(P<0.05).Conclusion 3D laparoscope combined with ERAS in the treatment of CRC can effectively shorten the operation time,accelerate the postoperative recovery of patients and reduce the incidence of complications,which is beneficial to improve the postoperative quality of life of patients.
