Application of GE Revolution CT in the diagnosis and treatment of intracranial aneurysms

摘要 目的探讨GE Revolution CT在颅内动脉瘤中的诊断价值。方法选取我院收治的高度疑为颅内动脉瘤的95例患者,均行GE Revolution CT与数字减影血管造影术(DSA)检查。以外科手术结果为“金标准”,评价GE Revolution CT检查与DSA检查对颅内动脉瘤的诊断价值。患者术后随访9~12个月,采用GE Revolution CT与DSA进行复查,评价颅内动脉瘤残留情况。结果GE Revolution CT检出阳性患者75例,颅内动脉瘤98个;DSA检出阳性患者77例,颅内动脉瘤100个;手术确诊阳性患者78例,颅内动脉瘤101个。GE Revolution CT与DSA检查颅内动脉瘤部位的符合率(97.03%vs.99.01%)比较,差异无统计学意义(P>0.05)。GE Revolution CT检出颅内动脉瘤的灵敏度、特异度、准确度、阳性预测值及阴性预测值分别为96.15%、100%、96.25%、100%和40.00%,与DSA的诊断结果比较差异无统计学意义(P>0.05)。GE Revolution C T检出术后颅内动脉瘤残留5个,DSA检出4个,两种方法检出颅内动脉残留比较差异无统计学意义(P>0.05)。结论GE Revolution CT与DSA诊断颅内动脉瘤均具有较高的灵敏度、特异度与准确度,且GE Revolution CT对术后动脉瘤残留亦具有良好的评估价值,临床可将GE Revolution CT作为颅内动脉瘤早期诊断及术后评估的重要影像学检查方法。 Objective To analyze the diagnostic value of GE Revolution CT in intracranial aneurysms.Methods A total of 95 patients with highly suspected intracranial aneurysms admitted to our hospital were selected,and all the patients were examined by GE Revolution CT and digital subtraction angiography(DSA).The diagnostic values of GE Revolution CT and DSA in intracranial aneurysms were evaluated by taking the surgical results as the“gold standard”.The patients were followed up for 9 to 12 months after operation,and re-examined by GE Revolution CT and DSA to evaluate the residual status of intracranial aneurysms.Results There were 75 positive cases and 98 intracranial aneurysms detected by GE Revolution CT,77 positive cases and 100 intracranial aneurysms detected by DSA,and 78 positive cases and 101 intracranial aneurysms found in the surgical operation.There was no statistically significant difference in the coincidence rate of intracranial aneurysm location by GE Revolution CT and DSA(97.03%vs.99.01%,P>0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of GE Revolution CT in the detection of intracranial aneurysms were 96.15%,100%,96.25%,100%and 40.00%respectively,there was no significant difference in the diagnostic results between GE Revolution CT and DSA(P>0.05).After operation,5 residual intracranial aneurysms were detected by GE Revolution CT and 4 by DSA,there was no significant difference in the detection rate of intracranial aneurysms between the two methods(P>0.05).Conclusion GE Revolution CT and DSA both have high sensitivity,specificity and accuracy in the diagnosis of intracranial aneurysms;besides,GE Revolution CT has good evaluation value for postoperative aneurysm residue,which can be used as an important imaging method for early diagnosis and postoperative evaluation of intracranial aneurysms in clinic.
