益气复脉注射液联合呋塞米治疗DCM心力衰竭患者的效果

Effect of Yiqi Fumai injection combined with furosemide for patients with dilated cardiomyopathy and heart failure

目的探讨益气复脉注射剂联合呋塞米治疗扩张型心肌病(DCM)心力衰竭患者的效果。方法采用随机对照研究。选取商丘市第一人民医院2019年6月至2022年3月收治的DCM心力衰竭患者98例,信封法随机分为对照组49例和观察组49例。对照组给予呋塞米治疗,观察组给予益气复脉注射液联合呋塞米治疗。比较两组患者的临床疗效及药物不良反应,所有患者均在治疗前及治疗后接受超声心电图扫描,测定心功能指标,采用免疫比浊法测定D二聚体(D-D)水平,免疫荧光定量法测定B型利钠肽原(BNP)水平,酶联免疫吸附法检测血管紧张素Ⅱ(ANGⅡ)水平。采用t、χ^(2)检验进行统计分析。结果观察组总有效率为95.92%(47/49),高于对照组的83.67%(41/49),两组比较,差异有统计学意义(P<0.05)。与治疗前比较,治疗后两组患者左心室射血分数(LVEF)水平均升高,且观察组高于对照组(均P<0.05);左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)水平均降低,且观察组均低于对照组(均P<0.05)。与治疗前比较,治疗后两组患者血浆D-D、BNP、ANGⅡ水平均下降,且观察组均低于对照组(均P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论DCM心力衰竭患者采用益气复脉注射液联合呋塞米治疗,能提高患者治疗总有效率,改善其心功能,降低D-D、BNP、ANGⅡ水平,具有较高的应用价值。

Objective To investigate the effect of Yiqi Fumai injection combined with furosemide for patients with dilated cardiomyopathy(DCM)and heart failure.Methods A total of 98 patients with DCM and heart failure admitted to Shangqiu People's Hospital from June 2019 to March 2022 were selected for the random control trial,and were randomly divided into a control group and an observation group by the envelope method,with 49 cases in each group.The control group were treated with furosemide,and the observation group with Yiqifumai injection and furosemide.The clinical efficacies of the two groups were compared.Before(T_(0))and after(T_(1))the treatment,all the patients underwent ultrasound electrocardiogram scanning;the indicators of cardiac function were measured;the D-dimer(D-D)level was measured by immunoturbidimetry;the level of pro-B-type natriuretic peptide(BNP)was detected by the immunofluorescence quantitative method;the level of angiotensinⅡ(ANGⅡ)was detected by enzyme-linked immunosorbent assay.The adverse drug reactions were compared between the two groups.t and χ^(2) tests were applied. Results The total effective rate of the observation group was higher than thatof the control group [95.92% (47/49) vs. 83.67% (41/49)], with a statistical difference betweenthese two groups (P<0.05). Compared with those at T_(0), the left ventricular ejection fractions (LVEF)at T_(1) in both groups were higher (both P<0.05);the LVEF at T_(1) in the observation group was higherthan that in the control group (P<0.05);the left ventricular end diastolic diameter (LVEDD) and leftventricular end systolic diameter (LVESD) at T_(1) were lower than those at T_(0) in both groups (all P<0.05);the LVEDD and LVESD in the observation group were lower than those in the control group(both P<0.05). Compared with those at T_(0), the plasma levels of D-D, BNP, and ANG Ⅱ at T_(1) werelower in the two groups (all P<0.05);the levels in the observation group were all lower than those inthe control group (all P<0.05). Conclusion Yiqi Fumai injection combined with furosemide forpatients with DCM and heart failure not only has a high total clinical effective rate, but also improvesthe patients' heart function and reduces the levels of DD, BNP and ANG Ⅱ, so it has high applicationvalue.
