多层螺旋电子计算机断层扫描技术诊断急性阑尾炎的价值

Value of Multi-Slice Spiral Computed Tomography Scan Technology in Diagnosing Acute Appendicitis

目的:探讨多层螺旋电子计算机断层扫描(CT)技术诊断急性阑尾炎的价值。方法:选取2020年1月—2022年1月湖北省监利市中医医院治疗的65例急性阑尾炎患者为研究对象,所有患者均经手术病理确诊,采用多层螺旋CT诊断,比较患者多层螺旋CT诊断结果与病理结果、CT分级与病理诊断结果。结果:多层螺旋CT与病理诊断的准确率比较,差异无统计学意义(P>0.05),其中1例将扩张增粗的阑尾误诊为末端回肠。CT分级0级1例,1级0例,2级29例,3级26例,4级5例,5级4例。而病理诊断为单纯性阑尾炎29例,占44.62%;诊断为阑尾炎伴阑尾周围炎26例,占40.00%;诊断为坏疽性或出血性阑尾炎伴穿孔5例,占7.69%;诊断为阑尾炎性包块或阑尾脓肿包块5例,占7.69%。结论:多层螺旋CT技术具有良好的密度分辨率及空间分辨率,在诊断急性阑尾炎方面可以多方位、多角度观察,扫描速度快,扫描时间短,诊断准确率高,具有较高的应用价值。

Objective:To explore the value of multi-slice spiral computed tomography(CT)scan technology in the diagnosis of acute appendicitis.Methods:A total of 65 patients with acute appendicitis treated in Jianli Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were selected as the study subjects.All patients were confirmed by surgery and pathology,and were diagnosed by multi-slice spiral CT.The multi-slice spiral CT diagnosis results and pathological results were were compared,and the CT grade and pathological diagnosis results were also were compared.Results:There was no significant difference in the accuracy of multi-slice spiral CT and pathological diagnosis(P>0.05).Among them,1 case of dilated and thickened appendix was misdiagnosed as terminal ileum.There were 1 case of CT Grade 0,0 case of CT Grade 1,29 cases of CT Grade 2,26 cases of CT Grade 3,5 cases of CT Grade 4,and 4 cases of CT Grade 5.29 cases were pathologically diagnosed with simple appendicitis,accounting for 44.62%;26 cases were pathologically diagnosed with appendicitis and periappendicitis,accounting for 40.00%;5 cases were pathologically diagnosed with gangrenous or hemorrhagic appendicitis with perforation,accounting for 7.69%;5 cases were pathologically diagnosed with appendicitis mass or abscess mass,accounting for 7.69%.Conclusion:Multi-slice spiral CT technology has good density resolution and spatial resolution,and can perform an observation from multiple directions and multiple angles in the diagnosis of acute appendicitis,with fast scanning speed,short scanning time,high diagnostic accuracy,and high application value.
