医养护一体化管理模式在社区老年慢性病患者管理中的应用价值

Application Value of Integrated Management Mode of Medical Care and Maintenance in Management of Elderly Patients with Chronic Diseases in Community

目的:探讨医养护一体化管理模式在社区老年慢性病患者管理中的应用价值。方法:选取2020年12月—2021年12月北京市顺义区第二医院收治的136例老年慢性病患者作为研究对象,根据管理模式差异分为常规组和试验组,各68例。常规组实施常规管理模式,试验组实施医养护一体化管理模式,比较两组干预前后自护能力评分、安全用药知识掌握情况与满意度。结果:干预前,两组对自身疾病的认识、自护技能、责任感、健康知识了解程度评分比较,差异无统计学意义(P>0.05);干预后,两组各项自护能力评分均高于干预前,且试验组高于常规组,差异有统计学意义(P<0.05)。干预前,两组对自身所用药物管理方式、基本认识、常见不良反应、用药情况评分比较,差异无统计学意义(P>0.05);干预后,两组各项安全用药知识掌握情况评分均高于干预前,且试验组高于常规组,差异有统计学意义(P<0.05)。试验组满意度高于常规组,差异有统计学意义(P<0.05)。结论:医养护一体化管理模式在社区老年慢性病患者管理中的应用效果显著,可有效提高患者自护能力、安全用药知识掌握程度与满意度,值得应用并予以推广。

Objective:To explore the application value of the integrated management mode of medical care and maintenance in the management of elderly patients with chronic diseases in the community.Methods:A total of 136 elderly patients with chronic diseases treated in the Second Hospital of Shunyi District of Beijing from December 2020 to December 2021 were selected as the study subjects.These patients were divided into routine group and experimental group according to the difference of management mode,with 68 cases in each group.The routine group implemented routine management mode and the experimental group implemented the integrated management mode of medical care and maintenance.The self-care ability score,safe medication knowledge mastery and satisfaction of the two groups before and after intervention were compared.Results:Before the intervention,there was no significant difference between the two groups in terms of their understanding with the disease,self-care skills,sense of responsibility and health knowledge mastery scores(P>0.05).After the intervention,the scores of self-care ability in two groups were higher than those before the intervention,and the experimental group was higher than the routine group,and the difference was statistically significant(P<0.05).Before the intervention,there was no significant difference between the two groups in terms of the drug management methods,basic knowledge,common adverse reactions and medication status scores(P>0.05).After the intervention,the scores of safe medication knowledge mastery in the two groups were higher than those before the intervention,and the experimental group was higher than the routine group,and the difference was statistically significant(P<0.05).The satisfaction in the experimental group was higher than that in the routine group,and the difference was statistically significant(P<0.05).Conclusion:The integrated management mode of medical care and maintenance in the management of elderly patients with chronic diseases in the community has a significant application effect,it can effectively improve the self-care ability of patients,the mastery of safe medication knowledge and the satisfaction,which is worthy of clinical application and promotion.
