血红蛋白在前列腺癌与前列腺增生中的对比分析

摘要 目的分析探讨血红蛋白在前列腺癌和前列腺增生患者血清中的水平,探讨其在前列腺癌中的诊断价值。方法回顾性分析2019年1月—2022年4月本院泌尿外科收治的139例前列腺癌和前列腺增生患者的临床资料,观察不同组别患者之间血红蛋白水平的差异。结果经Mann-Whitney U非参数检验,前列腺增生患者血红蛋白水平显著高于前列腺癌患者(P<0.001)。与前列腺增生组患者相比,前列腺癌骨转移亚组以及前列腺癌病理分级亚组血红蛋白水平更低(P<0.001)。Spearman相关性检验显示,前列腺癌患者的病理分级、是否骨转移与血红蛋白水平具有显著负相关(P<0.001)。将血红蛋白用于前列腺癌诊断,其曲线下面积为0.632,其取值为137.5g/L时,灵敏度为61.9%,特异性为58.6%。在诊断前列腺癌骨转移时,其曲线下面积为0.736,当血红蛋白取值为128.5g/L时,灵敏度为67.6%,特异性为75.7%。结论治疗前血红蛋白水平对于前列腺癌有一定诊断价值,可作为其辅助诊断指标之一。 Objective To analyze the level of hemoglobin in the serum of patients with prostate cancer and benign prostatic hyperplasia,and to explore it’s diagnostic value in prostate cancer.Methods The clinical data of 139 patients with prostate cancer(PCa)and benign prostatic hyperplasia(BPH),those were admitted to urinary surgery department of the hospital during January 2019 and April 2022,were retrospectively analyzed.Results According to the results of Mann-Whitney U non-parametric test,the hemoglobin level in patients with BPH was significantly higher than that in patients with prostate cancer(P<0.001).The hemoglobin levels of the prostate cancer bone metastasis subgroup and the prostate cancer pathological grade subgroup were lower THAN the patients with benign prostatic hyperplasia,the difference was statistically significant(P<0.001).Spearman correlation test showed that there was a significant negative correlation between the pathological grade,bone metastasis and hemoglobin level in patients with prostate cancer(P<0.001).When hemoglobin was used in the diagnosis of prostate cancer,the area under the curve was 0.632,and the cut off value was 137.5g/l,the sensitivity was 61.9%and the specificity was 58.6%.When diagnosing bone metastasis of prostate cancer,the area under the curve was 0.736,when hemoglobin was 128.5g/l,the sensitivity was 67.6%and the specificity was 75.7%.Conclusions The hemoglobin level before treatment has a certain diagnostic value for prostate cancer and could be used as one of its auxiliary diagnostic indicators.
