SNAPPE-Ⅱ评分联合肺超声评分在新生儿呼吸窘迫综合征预后评估中的价值

The value of SNAPPE-Ⅱscore combined with lung ultrasound score in prognostic evaluation of neonatal respiratory distress syndrome

目的分析SNAPPE-Ⅱ评分联合肺超声评分在新生儿呼吸窘迫综合征(NRDS)预后评估中的价值。方法选取2018年1月—2021年6月本院收治的NRDS患儿82例的临床资料进行回顾性分析。所有患儿治疗前均接受肺超声检测和新生儿急性生理学评分围产期补充Ⅱ(SNAPPE-Ⅱ)评分评估。统计NRDS患儿治疗后死亡情况。采用Pearson相关性分析法分析NRDS患儿肺超声评分和SNAPPE-Ⅱ评分相关性。采用ROC曲线下面积分析肺超声评分、SNAPPE-Ⅱ评分及二者联合检测在NRDS预后中的诊断价值。结果82例NRDS患儿中,治疗后死亡37例设为死亡组,治疗后生存45例设为生存组。两组患儿一般资料对比无显著差异(P>0.05)。死亡组患儿肺超声评分为(38.25±5.64)分,SNAPPE-Ⅱ评分为(27.81±7.45)分,均显著高于生存组(P<0.05)。NRDS患儿肺超声评分与SNAPPE-Ⅱ评分呈正相关(r=0.615,P<0.05)。肺超声评分+SNAPPE-Ⅱ评分联合检测显著优于二者单独检测(P<0.05)。结论肺超声评分与SNAPPE-Ⅱ评分越高NRDS患儿的死亡风险越高,两项评分对NRDS预后诊断均具有较好的价值,且二者联合检测效果更好。

Objective To analyze the value of SNAPPE-Ⅱscore combined with lung ultrasound score in prognostic evaluation of neonatal respiratory distress syndrome(NRDS).Methods The clinical data of 82 children with NRDS those were admitted to our hospital from January 2018 to June 2021 were selected for retrospective analysis.All infants received lung ultrasonography and neonatal acute physiology score perinatal supplement II(SNAPPE-II)score evaluation before treatment.Statistics of deaths in children with NRDS after treatment.Pearson correlation analysis was used to analyze the correlation between lung ultrasound score and SNAPPE-Ⅱscore in children with NRDS.The area under the ROC curve was used to analyze the diagnostic value of lung ultrasound score,SNAPPE-Ⅱscore and their combined detection in the prognosis of NRDS.Results Among the 82 children with NRDS,37 children who died after treatment were set as the death group,and 45 children who survived after treatment were set as the survival group.There was no significant difference in general data between the two groups(P>0.05).The lung ultrasound score(38.25±5.64)and SNAPPE-Ⅱscore(27.81±7.45)in the death group were significantly higher than those in the survival group(P<0.05).The lung ultrasound score of children with NRDS was positively correlated with SNAPPE-Ⅱscore(r=0.615,P<0.05).The combined detection of lung ultrasound score and SNAPPE-Ⅱscore was significantly better than that of the two alone(P<0.05).Conclusions The higher the lung ultrasound score and the SNAPPE-Ⅱscore,the higher the risk of death in NRDS children.Both scores have good value in the prognostic diagnosis of NRDS,and the combined detection effect of the two will be better.
