某院全面推进全生命周期健康管理工作实践与思考

The practice and thinking of a hospital to comprehensively promote the health management of the whole life cycle

全生命周期健康管理是为民众提供集预防、治疗、康复、健康管理为一体的健康服务,是实现疾病诊治向疾病防控转变的重要举措。3年来,德阳市人民医院积极探索建立适合国情、域况、医院和民众需要的全生命周期健康管理体系。通过制度研究、信息化建设、诊疗模式构建、流程改造等举措,从解决人民群众看病就医问题向促进和改善人民健康转变,逐步实现全方位、全周期维护和保障人民健康,落实“以人民健康为中心”的健康中国战略。

Life-cycle health management is a medical resource reorganization that integrates prevention,treatment,rehabilitation,and health management for the whole people,and it is an important measure to transform from the pattern of diagnosis and treatment to the pattern of prevention and control.In the past three years,Deyang People's Hospital has actively explored ways to set up a life-cycle health management system that adapts to national and regional conditions and meets the needs of people and the hospital.Starting with measures of institutional research,information construction,model construction of diagnosis and treatment and process improvement,etc.,the hospital has transformed into promoting and improving people's health more than solving the problem of improving medical services.We will gradually realize the goal of protecting people’s health in all-around and whole-cycle ways and carry out the healthy China initiative of"putting people's health first".
