补虚生肌方治疗痔瘘术后创面不愈合及感染的临床观察

Clinical efficacy of Buxu Shengji Formula for postoperative unhealed wound surface and infection in patients after hemorrhoid fistula operation

目的观察补虚生肌方治疗痔瘘术后创面不愈合及感染的临床疗效。方法将200例痔瘘术后患者按照随机数字表法分为2组,对照组100例术后予常规治疗,治疗组100例在对照组治疗基础上加补虚生肌方治疗。2组均治疗14天。比较2组疗效;比较2组治疗前后创面情况;比较2组治疗前后疼痛视觉模拟评分(VAS)、5-羟色胺(5-HT)、P物质(SP)、神经肽Y(NPY)变化;比较2组治疗前后炎症指标变化;比较2组创面愈合时间、住院时间。结果治疗组总有效率95.00%(95/100),对照组总有效率80.00%(80/100),治疗组疗效优于对照组(P<0.05)。2组治疗后创面面积、创面分泌物评分、创面肉芽形成评分、创面出血评分、创缘水肿评分均较本组治疗前降低(P<0.05),治疗后治疗组创面面积、创面分泌物评分、创面肉芽形成评分、创面出血评分、创缘水肿评分均低于对照组(P<0.05)。2组治疗后疼痛VAS、5-HT、SP、NPY水平均较本组治疗前降低(P<0.05),治疗后治疗组疼痛VAS、5-HT、SP、NPY水平均低于对照组(P<0.05)。2组治疗后肿瘤坏死因子α(TNF-α)、白细胞介素8(IL-8)、C反应蛋白(CRP)水平均较本组治疗前降低(P<0.05),治疗后治疗组TNF-α、IL-8、CRP水平均低于对照组(P<0.05)。治疗组创面愈合时间、住院时间均较对照组提前(P<0.05)。结论补虚生肌方治疗痔瘘术后患者,能促进创面愈合,减少术后创面感染,缩短疗程。

Objective To observe the clinical efficacy of Buxu Shengji Formula for postoperative unhealed wound surface and infection in patients after hemorrhoid fistula operation.Methods Totally 200 patients after hemorrhoid fistula operation were randomly assigned to receive either conventional therapy(the control group,n=100)or conventional therapy+Buxu Shengji Formula(the treatment group,n=100).A 14-day treatment was performed to compare wound condition,visual analogue scale(VAS)for pain,5-hydroxytryptamine(5-HT),substance P(SP),neuropeptide Y(NPY),inflammatory indexes,wound healing time,hospitalization time.The curative effect was assessed.Results The overall effective rate was 95.00%(95/100)in the treatment group,higher than that of 80.00%(80/100)in the control group(P<0.05).After treatment,the scores of wound area,wound secretion,wound granulation growth,wound bleeding,wound edema in groups were decreased(P<0.05),which were lower in the treatment group than in the control group(P<0.05).VAS,5-HT,SP,NPY in groups after treatment were decreased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05).After treatment,tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),C-reactive protein(CRP)in groups were decreased(P<0.05),the reduction in the treatment group was more common than the control group(P<0.05).The wound healing time and hospitalization time were shorter in the treatment group than in the control group(P<0.05).Conclusion Buxu Shengji Formula for patients after hemorrhoid fistula operation can promote wound healing,reduce postoperative wound infection,and shorten course of treatment.
