妊娠期高血压疾病患者终止妊娠时机及分娩方式临床分析

Clinical analysis of pregnancy termination time and delivery mode in hypertensive diseases during pregnancy

目的 探讨妊娠期高血压疾病患者终止妊娠时机及分娩方式。方法 选取60例妊娠期高血压疾病患者,根据终止妊娠时机分组,孕周<36周者24例,孕周≥36周者36例;根据分娩方式分组,经阴道分娩22例,剖宫产38例。对比母婴结局。结果 孕周<36周和孕周≥36周患者并发症发生率比较,差异无统计学意义(P>0.05),孕周≥36周新生儿窒息发生率8.33%、围产儿死亡率2.78%低于孕周<36周的41.67%、33.33%,差异具有统计学意义(P<0.05)。经阴道分娩患者并发症发生率、新生儿窒息发生率及围产儿死亡率高于剖宫产患者,差异具有统计学意义(P<0.05)。结论 妊娠期高血压疾病患者孕周较短时,可将孕周延长至36周,以改善预后;采用剖宫产分娩方式,可防范并发症。

Objective To explore the time of termination of pregnancy and delivery mode of hypertensive diseases in pregnancy.Methods 60 patients with gestational hypertension were selected and grouped according to the time of termination of pregnancy.24 patients with gestational age<36 weeks and 36 patients with gestational age 236 weeks.According to the method of delivery,there were 22 cases of vaginal delivery and 38 cases of cesarean section.Compare maternal and infant outcomes.Results There was no significant difference in the incidence of complications between patients with gestation weeks<36 weeks and those with gestation weeks$36 weeks(P>0.05).The incidence of neonatal asphyxia 8.33%and perinatal mortality 2.78% of patients with gestation weeks$36 weeks were lower than those of 41.67% and 33.33% of patients with gestation weeks<36 weeks(P<0.05).The incidence of complications,neonatal asphyxia and perinatal mortality in patients with vaginal delivery was higher than that in patients with cesarean section,and the difference was statistically significant(P<0.05).Conclusion For the patients with pregnancy-induced hypertension,the gestational age is shorter,the gestational age can be extended to 36 weeks to improve the prognosis.Caesarean delivery can prevent complications.
