摘要
目的探究增强T_(2)*加权血管成像(ESWAN)参数评估子宫内膜癌(EC)血管成熟度的价值。方法回顾性收集70例经手术病理证实为EC的病人,平均年龄(55.0±10.5)岁。所有病人均于术前行盆腔常规MRI及ESWAN检查,并测量ESWAN参数值。采用免疫荧光染色法标记EC细胞微血管密度(MVD)、微血管周细胞密度(MPD),并计算微血管周细胞覆盖率(MPI),以评估肿瘤血管成熟度。按照肿瘤组织分化程度将病人分为高分化组(35例)及中分化(35例)组,2组肿瘤病理组织切片中细胞数目均为175个。采用独立样本t检验比较2组间血管成熟度相关指标及ESWAN参数的差异,采用Pearson相关分析ESWAN各参数与MPI的相关性。结果高分化细胞中的MPI高于中分化细胞(P<0.05),高、中分化细胞中的MVD、MPD差异均无统计学意义(均P>0.05)。中分化组的相位值、R_(2)*值高于高分化组,T_(2)*值低于高分化组(均P<0.05)。2组幅度值的差异无统计学意义(P>0.05)。EC的MPI与R_(2)*值呈负相关(r=-0.645,P=0.013),与T_(2)*值呈正相关(r=0.543,P=0.045);与幅度值、相位值均无相关性(均P>0.05)。结论ESWAN序列R_(2)*值、T_(2)*值对评估EC血管成熟度有一定价值;R_(2)*值越大,EC血管成熟度越低;T_(2)*值越大,EC血管成熟度越高。
Objective To investigate the value of parameters of enhanced T_(2)*weighted angiography(ESWAN)in assessing vascular maturity in endometrial cancer(EC).Methods Seventy patients(55.0±10.5 years old)with EC confirmed by surgery and pathology were retrospectively collected.All patients underwent conventional pelvic MRI and ESWAN examination before operation,and the parameters of ESWAN were measured from each lesion.Microvascular density(MVD)and perivascular cell density(MPD)of EC cells were labeled by immunofluorescence staining,and perivascular cell coverage(MPI)was calculated to evaluate tumor vascular maturity.According to the degree of tumor tissue differentiation,the patients were divided into highly differentiated group(35 cases)and moderately differentiated group(35 cases).The number of cells in the pathological tissue slices of both groups was 175.Independent sample t test was used to compare the differences in vascular maturity indexes and ESWAN parameters between the two groups,and Pearson correlation was used to analyze the correlation between ESWAN parameters and MPI.Results The MPI in highly differentiated cells was higher than that in moderately differentiated cells(P<0.05),but there was no significant difference in MVD and MPD between highly and moderately differentiated cells(P>0.05).The phase and R_(2)*values of the moderately differentiated group were higher than those of the highly differentiated group,while the T_(2)*values were lower than those of the highly differentiated group(all P<0.05).There was no significant difference in amplitude between the two groups(P>0.05).The MPI of EC was negatively correlated with R_(2)*value(r=-0.645,P=0.013),and positively correlated with T_(2)*value(r=0.543,P=0.045),but had no correlation with amplitude value and phase value(P>0.05).Conclusion The values of R_(2)*and T_(2)*of the ESWAN sequence were of certain value for evaluating EC vascular maturity.The higher the R_(2)*value,the lower the EC vascular maturity,and the higher the T_(2)*value,the higher the EC vascular maturity.
