The classic MRI total burden scoring system of cerebral small vessel disease(CSVD)can be used to capture the whole brain injury due to CSVD by incorporating a combination of four imaging features:white matter hyperintensity,lacuna,perivascular space and cerebral microbleed.Standardized assessment of CSVD total burden provides rapid quantification or stratification of CSVD and has been applied in clinical research,clinical trials and everyday clinical practice.An overview of CSVD total burden score,its clinical application in the assessment of cognition,stroke,affective disorders and motor disorders,improvement options for the classical scale,and advantages of the scale and its future application are reviewed.
