雄激素受体在不同分子亚型乳腺癌中的表达及意义

摘要 目的 探讨不同分子亚型乳腺癌中雄激素受体(AR)的表达及其临床意义。方法 选取新乡市第一人民医院2018年2月至2021年2月行手术治疗的135例乳腺癌患者为研究对象,采用免疫组织化学法检测不同分子亚型乳腺癌组织中AR表达情况。分析AR表达与临床病理参数、不同分子亚型、预后的关系。结果 乳腺癌组织中AR阳性率(80.00%)低于癌旁组织AR阳性率(90.37%),差异有统计学意义(P<0.05);雌激素受体(ER)、孕激素受体(PR)阳性者AR阳性率分别高于ER、PR阴性者,Luminal A型、Luminal B型者AR阳性率分别高于人表皮生长因子受体2(HER-2)过表达型、三阴型者,差异有统计学意义(P<0.05);AR阳性率与ER、PR表达呈正相关,而与临床分期、组织学分级、腋窝淋巴结转移呈负相关,差异有统计学意义(P<0.05);AR阳性者生存率(82.35%)高于AR阴性者(54.55%),差异有统计学意义(P<0.05)。结论 Luminal A型、Luminal B型乳腺癌患者AR阳性率相对更高,AR表达与不同分子亚型、临床病理参数存在相关性,且AR阳性者预后较好。 Objective To explore the different molecular subtypes of breast cancer in the expression of androgen receptor(AR)and its clinical significance.Methods A total of 135 patients with breast cancer who underwent surgery in the First People's Hospital of Xinxiang from February 2018 to February 2021 were selected as the research objects.The expression of AR in different molecular subtypes of breast cancer tissues was detected by immunohistochemistry.The relationship between AR expression and clinicopathologic parameters,different molecular subtypes,and prognosis were analyzed.Results The positive rate of AR in breast cancer tissues(80.00%)was lower than that in adjacent tissues(90.37%),and the difference was statistically significant(P<0.05).The AR-positive rates of estrogen receptor(ER)and progesterone receptor(PR)positive patients were higher than those of ER and PR-negative patients,and the AR-positive rates of Luminal A and Luminal B types were higher than those of human epidermal growth factor receptor 2(HER-2)over-expression and triple-negative patients,respectively,and the differences were statistically significant(P<0.05).The positive rate of AR was positively correlated with the expression of ER and PR,but negatively correlated with clinical stage,histological grade,and axillary lymph node metastasis,with statistical significance(P<0.05).The survival rate of AR-positive patients(82.35%)was higher than that of AR-negative patients(54.55%),and the difference was statistically significant(P<0.05).Conclusion The positive rate of AR is relatively higher in Luminal A and Luminal B breast cancer patients,and AR expression is correlated with different molecular subtypes,and clinicopathological parameters,and AR-positive patients have a better prognosis.
