摘要
目的 探讨细菌性血流感染早期诊断中血培养联合血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)水平检测的应用价值。方法 选取2019年6月至2022年6月在河南中医药大学人民医院接受治疗的58例细菌性血流感染患者,将其命名为感染组(N=58),另选取58名健康体检者,将其命名为对照组(N=58),比较不同组别、不同细菌种类的PCT、hs-CRP水平,分析不同检测方式的诊断价值。结果 感染组PCT、hs-CRP水平较对照组更高,差异有统计学意义(P<0.001);感染革兰氏阳性菌患者PCT水平较感染革兰氏阴性菌患者更低,差异有统计学意义(P<0.001);血培养联合血清PCT、hs-CRP水平检测的灵敏度、特异度、阳性预测值与阴性预测值比单一检测方式相更高,差异有统计学意义(P<0.001)。结论 血培养联合血清PCT、hs-CRP水平检测在早期诊断细菌性血流感染中的应用能够提高检出率,更有利于后续治疗。
Objective To investigate the application value of blood culture combined with serum procalcitonin(PCT)and hypersensitive C-reactive protein(hs-CRP)levels in the early diagnosis of bacterial bloodstream infection.Methods A total of 58 patients with bacterial bloodstream infection who were treated at People's Hospital of Henan University of Traditional Chinese Medicine from June 2019 to June 2022 were selected and named as the infection group(N=58).Another 58 healthy subjects were selected and named as the control group(N=58).The levels of PCT and hs-CRP in different groups and different bacterial species were compared to analyze the diagnostic value of different detection methods.Results The levels of PCT and hs-CRP in the infection group were higher than those in the control group,and the difference was statistically significant(P<0.001).The PCT level of patients infected with Gram-positive bacteria was lower than that of patients infected with Gram-negative bacteria,and the difference was statistically significant(P<0.001).The sensitivity,specificity,positive predictive value,and negative predictive value of blood culture combined with serum PCT and hs-CRP levels were higher than those of single detection,and the difference was statistically significant(P<0.001).Conclusion The application of blood culture combined with serum PCT and hs-CRP in the early diagnosis of bacterial bloodstream infection can improve the detection rate and is more conducive to subsequent treatment.
