孔光一辨治温病护阳思想解析

Analysis of KONG Guang-yi’s thought of protecting yang qi when treating warm disease

中医理论中重阳的思想起源于《黄帝内经》,温病的治疗偏重于清热和养阴,不应忽略对阳气的顾护,孔光一教授是现代温病学家,国家级名老中医、首都国医名师,他临床70余载,辨治温病或杂病之热证疗效显著,注意清热而不伤阳气,养阴而不敛邪,护阳思想贯穿始终。笔者结合临床跟诊学习心得总结孔光一教授通阳、护阳、温阳思想在治疗热证中的使用要点:邪郁不达,病理产物,通阳却之;慎用大寒,阴生阳长,注意护阳;辨识素体,医嘱到位,益气温阳;阳之或通或护或温,诸法共融,治疗时往往寒温并用,润燥相合,以平为期。总结了孔光一教授辨治温病时护阳思想和具体临证经验,启示后学。

Professor KONG Guang-yi is known as one of the famous national veteran traditional Chinese medicine experts with 70 years of clinical experience,and good at treating warm disease.The thought of protecting yang qi originates from Huangdi Neijing,however,it has always been overlooked when clearing heat and nourishing yin during treating warm disease,professor KONG takes account of yang qi when clearing heat and nourishing yin by using cold nature medications.This article summarizes the key points of professor KONG Guang-yi’s treatment of activating yang qi,protecting yang qi,and warming yang qi in the course of the warm disease based on the clinical follow-up study experience:Use the method of activating yang qi to remove the qi stagnation and pathological products;Be careful with strong cold medicine,yin grows while yang grows,and pay attention to protecting yang qi;Identify the nature of body,keep the doctor’s advice be proper,boost qi and warm yang qi;Treatment should be a combination of activating yang qi,protecting yang qi,and warming yang qi.In the treatment,the combination of cold medicine and warm medicine is often used,and moisturizing medicine and drying medicine are combined to achieve the purpose of balance.This article summarized professor KONG Guang-yi’s thoughts and specific clinical experience of protecting yang qi in the diagnosis and treatment of febrile diseases,and enlightened future study.
