针刺阿是穴对腰肌筋膜疼痛综合征多裂肌形态和功能的影响

Effects of acupuncture at Ashi point on the morphology and function of multifidus muscle in psoas myofascial pain syndrome

目的:观察针刺阿是穴治疗腰肌筋膜疼痛综合征对多裂肌形态和功能的影响。方法:采用随机对照试验设计,将60例腰肌筋膜疼痛综合征患者随机分为治疗组及对照组,每组30例;治疗组采用阿是穴针刺治疗;对照组采用常规穴位针刺治疗;两组均隔日治疗1次,每周3次,共4周,观察患者治疗前后疼痛视觉模拟评分(VAS)、压力痛阈(PPT)、腰椎功能障碍评估(JOA)、软组织张力检测及多裂肌的横截面积变化。结果:与治疗前比较,治疗后两组患者的VAS评分降低、PPT值升高、软组织位移距离值增加、JOA评分增加、多裂肌横截面积增大(P<0.05),且治疗组均显著优于对照组(P<0.05),治疗组阿是穴肌肉结节消失。结论:阿是穴刺激可消除多裂肌痉挛,改善腰肌筋膜疼痛综合征患者的疼痛症状和腰椎功能,疗效优于常规穴位针刺治疗。

To observe the effect of acupuncture at Ashi point on the morphology and function of multifidus in the treatment of psoas myofascial pain syndrome.Methods:Using a randomized controlled trial design,60 patients with psoas myofascial pain syndrome were randomly divided into a treatment group and a control group,30 cases in each group.The treatment group was treated with acupuncture at Ashi points;the control group was treated with acupuncture at conventional acupoints treatment.Both groups were treated once every other day,3 times a week,for a total of 4 weeks,and the pain visual analogue scale(VAS),pressure pain threshold(PPT),lumbar spine dysfunction assessment(JOA),soft tissue tension detection and detection before and after treatment were observed.Changes in the cross-sectional area of the multifidus muscle.Results:Compared with before treatment,after treatment,the VAS score decreased,PPT value increased,soft tissue displacement distance increased,JOA score increased,and multifidus cross-sectional area increased in the two groups,which were significantly different from those before treatment(P<0.05).The treatment group was significantly better than the control group after treatment(P<0.05).In the treatment group,the nodule of Ashi point disappeared.Conclusion:Ashi acupoint stimulation can eliminate multifidus spasm,improve pain symptoms and lumbar spine function in patients with psoas myofascial pain syndrome,and the curative effect is better than conventional acupuncture treatment.
