摘要
慢性肾脏病(CKD)是由各种原发性及继发性肾脏疾病导致肾脏结构和功能损伤所引起的一系列临床症状的综合征。由于代谢紊乱和内环境失衡的影响,CKD患者常易并发睡眠障碍。从中医角度分析,其发病与肝关系密切。肝肾同源,肾精不足则不能化生濡养肝血;肾亏日久,浊邪内壅导致少阳枢机不利;以及肾虚久病进一步诱发和加重肝郁,以上因素均可引发和加重不寐的发生。本文总结笔者导师王明教授临床心得,从补肾不忘补肝、清郁热以畅枢机、调气血以安肝魂3个方面介绍从肝论治CKD失眠的经验,并附医案1则,以期为同道辨治CKD失眠提供新的思路。
Chronic kidney disease is a syndrome with renal structural and functional impairment caused by various primary and secondary kidney diseases.Due to metabolic disorders and homeostatic imbalance,patients with chronic kidney disease are often complicated by sleep disorders.In traditional Chinese medicine(TCM),chronic kidney disease is closely related to the liver.Specifically,since liver and kidney are homologous,kidney essence deficiency results in the failure to transform,generate and replenish liver blood,and persistent kidney depletion causes internal stagnation of turbid pathogen,thereby leading to unfavorable Shaoyang pivot;in addition,chronic diseases caused by kidney deficiency further induce and aggravate liver depression.All the above factors can cause and exacerbate insomnia.This article summarized the clinical experience of Supervisor Wang in treating insomnia in chronic kidney disease from three aspects:tonifying liver while tonifying kidney,clearing stagnated heat to smooth the pivot,and regulating qi and blood to tranquilize liver soul,with one medical record attached.It aimed to provide new ideas for differentiation and treatment of insomnia in chronic kidney disease.
