依托咪酯联合丙泊酚在老年无痛胃肠镜检查中的应用效果分析

Effect analysis of etomidate combined with propofol in painless gastroenteroscopy in the elderly

目的探讨依托咪酯联合丙泊酚对行无痛胃肠镜检查的老年研究对象的麻醉效果,并分析其对收缩压(SBP)、舒张压(DBP)和心率(HR)的影响。方法选取2021年9月至2022年9月德阳市人民医院收治的100例接受无痛胃肠镜检查老年研究对象进行研究,按照随机数字表法分为对照组和研究组,各50例。对照组研究对象麻醉方式为单用丙泊酚,研究组研究对象在对照组基础上加用依托咪酯。比较两组研究对象的麻醉效果,观察两组研究对象的血压变化、HR、血氧饱和度(SaO2)变化及相关认知功能指标。结果研究组研究对象的麻醉诱导时间、苏醒时间、定向力恢复时间短于对照组(P<0.05);研究组研究对象在麻醉后30 min的简易精神状态评价量表(MMSE)评分高于对照组(P<0.05);麻醉后5 min研究组研究对象的SBP、DBP水平高于对照组(P<0.05);两组研究对象不良反应发生率比较,差异无统计学意义(P>0.05)。结论老年人群在进行无痛胃肠镜检查时,可将依托咪酯与丙泊酚的联合方案作为首选,麻醉起效快,对血压影响小,安全性较高,麻醉后恢复效果也相对理想,具有临床应用价值。基金康盟基金会-医学与健康事业研究发展基金项目(编号:S025)

Objective To investigate the effect of combined anesthesia with etomidate and propofol on the elderly undergoing painless gastroscopy,and analyze its effects on systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR).Methods 100 elderly subjects with painless gastroscopy admitted to Deyang People’s Hospital from September 2021 to September 2022 were selected as the research objects,and were divided into the control group and the study group according to the random number table method,with 50 cases in each group.Research object in the control group were anesthetized with propofol alone,and research object in the study group were treated with etomidate on the basis of the control group.The anesthetic effects of the two groups were compared,and the changes of blood pressure,HR,oxygen saturation of blood(SaO2)and related cognitive function indexes were observed.Results The time of anesthesia induction,awakening and recovery of orientation in the study group were shorter than those in the control group(P<0.05).The mini-mental state examination(MMSE)scores in the study group were higher than those in the control group 30 minutes after anesthesia(P<0.05).The levels of SBP and DBP of the study group were significantly higher than those of the control group 5 minutes after anesthesia(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of etomidate and propofol can be the first choice for elderly patients during painless gastroscopy.The anesthesia has quick effect,little impact on blood pressure,high safety,and relatively ideal recovery effect after anesthesia,which has clinical application value.
