摘要
目的:分析机械通气重症肺炎患者行早期渐进性活动联合呼吸康复的临床效果。方法:选取2020年1月-2022年1月佛山市第五人民医院收治的采用机械通气的重症肺炎患者100例为观察对象,根据基础资料均衡可比的原则分为对照组(n=48)与观察组(n=52)。对照组给予呼吸康复治疗,观察组在对照组基础上联合早期渐进性活动。比较两组患者机械通气时间与ICU停留时间、炎性水平[白细胞计数(WBC)、C反应蛋白(CRP)水平和白细胞介素-6(IL-6)]、临床效果。结果:观察组机械通气时间、ICU停留时间短于对照组,差异有统计学意义(P<0.001);治疗后,两组患者WBC、CRP、IL-6水平均下降,观察组低于对照组,差异有统计学意义(P<0.001);观察组治疗总有效率高于对照组,差异有统计学意义(P=0.005)。结论:机械通气重症肺炎患者实施早期渐进性活动联合呼吸康复可显著缓解患者临床症状,缩短机械通气时间、ICU停留时间,改善患者炎性因子水平。
Objective:To analyze the clinical effect of early progressive activity combined with respiratory rehabilitation in severe pneumonia patients during mechanical ventilation.Methods:A total of one hundred patients with severe pneumonia treated with mechanical ventilation in the 5th People's Hospital of Foshan City from January 2020 to January 2022 were selected as the observation subjects,and were divided into control group(n=48)and observation group(n=52)according to the principle of balanced and comparable basic data.The control group was given respiratory rehabilitation,and the observation group was given early progressive activities on the basis of the control group.Mechanical ventilation duration,ICU stay time,inflammatory factor levels[white blood cell count(WBC),C-reactive protein(CRP)and interleukin-6(IL-6)]and clinical effect were compared between the two groups.Results:The mechanical ventilation time and ICU stay time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.001).After treatment,the levels of WBC,CRP and IL-6 in two groups were decreased,and the levels in the observation group were lower than those in the control group,with statistically significant differences(P<0.001).The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.005).Conclusion:In severe pneumonia patients with mechanical ventilation,early progressive activities combined with respiratory rehabilitation can significantly relieve clinical symptoms,shorten the time of mechanical ventilation and ICU stay time,and improve the levels of inflammatory factors in patients.
