老年慢性病共病患者参与药物管理体验的现象学研究

Experiences of Participation in Medication Management among Elderly Patients with Multiple Chronic Conditions:A Phenomenological Study

目的 探讨老年慢性病共病患者参与药物管理体验,为制订药物教育方案提供参考。方法 采用目的抽样法选取17例老年慢性病共病患者为研究对象,对其进行半结构式访谈,采用现象学分析方法Colaizzi 7步分析法对资料进行整理分析。结果 共归纳出4个主题:参与态度积极、参与能力匮乏、过渡期不适应、信息沟通不充分。结论 老年慢性病共病患者药物管理体验多样,护理人员应综合考虑患者的认知能力及心理变化,提供个性化指导,引导患者充分利用社会支持,参与自身药物管理。

Objective To explore the experiences of participation in medication management among elderly patients with multiple chronic conditions,so as to provide references for developing medication training programs.Methods Purposive sampling was used to select 17 elderly patients with multiple chronic conditions for semi-structured interviews.The Colaizzi 7-step analysis,a phenomenological analysis method,was adopted to analyze the data.Results Four themes were summarized,including active participation attitude,lack of participation ability,maladaptation in transitional period,insufficient information communication.Conclusions There are diverse experiences of medication management among elderly patients with multiple chronic conditions.Nurses should provide individualized instructions considering the patients' cognitive abilities and psychological changes.Nurses should take full advantage of social support to guide patients to participate in their own medication management.
