早期ONS支持对膀胱癌患者术后营养状况和机体恢复的影响

Effect of early ONS supportive care on postoperative nutritional status and recovery of patients with bladder cancer

目的探讨早期口服营养补充(ONS)支持护理对膀胱癌患者术后营养状况和机体恢复的影响。方法选取2020年1月—2022年4月郑州大学第一附属医院收治的84例膀胱癌患者,采用随机数字表法分为研究组与对照组,每组42例。2组患者均行根治性膀胱全切术治疗,对照组患者术后采用肠外营养支持,研究组患者术后采用早期ONS支持。比较2组患者术后恢复情况,干预前后营养状况、癌因性疲乏程度及术后10 d内胃肠道不良反应发生情况。结果研究组患者干预后7 d血清白蛋白、血红蛋白、前清蛋白水平均高于对照组,差异均有统计学意义(P<0.05)。研究组患者营养支持时间、下床活动时间、住院时间均短于对照组,差异均有统计学意义(P<0.05)。研究组患者干预后1个月Prper疲乏量表中行为维度、情感维度、感知维度、认知维度评分均低于对照组,差异均有统计学意义(P<0.05)。研究组患者胃肠道不良反应发生率为14.29%(6/42),低于对照组的33.33%(14/42),差异有统计学意义(χ^(2)=4.200,P=0.040)。结论早期ONS支持护理用于膀胱癌术后患者,有助于改善患者营养状况,促进患者恢复,减轻患者癌因性疲乏程度,且胃肠道不良反应发生风险较低,应用价值良好。

Objective To investigate the effect of early oral nutritional supplements(ONS)supportive care on postoperative nutritional status and recovery of patients with bladder cancer.Methods From January 2020 to April 2022,84 patients with bladder cancer were randomly assigned to the study group(n=42)and the control group(n=42).Patients in both groups received radical cystectomy.Patients in the control group received parenteral nutrition support after surgery and patients in the study group received early ONS support care after surgery.Postoperative recovery,nutritional status before and after the intervention,degree of cancer-related fatigue,and incidence of gastrointestinal adverse reactions within 10 days after surgery were compared between the two groups.Results The levels of serum albumin,hemoglobin and prealbumin in the study group were higher than those in the control group 7 days after the intervention,and the differences were statistically significant(P<0.05).The nutritional support time,ambulation time,and hospital stay in the study group were shorter than that in the control group,with statistical significance(P<0.05).The scores of behavioral,affective,sensory,and cognitive dimensions in the Piper Fatigue Scale(PFS)in the study group were lower than that in the control group 1 month after the intervention,with statistical significance(P<0.05).The incidence rate of gastrointestinal adverse reactions in the study group was 14.29%(6/42),which was lower than that(33.33%,14/42)in the control group,with statistical significance(χ^(2)=4.200,P=0.040).Conclusion Early ONS support care for patients with bladder cancer after the operation is helpful to improve their nutritional status,promote their recovery,and reduce the degree of cancer-related fatigue,with a low risk of gastrointestinal adverse reactions.These benefits hold promise for a wider application of this technique.
