摘要
目的探讨不同听阈曲线类型突发性聋患者的血脂、椎-基底动脉血流动力学特征。方法回顾性选取2020年5月至2022年5月于驻马店市第一人民医院接受检查的89例突发性聋患者纳入观察组,根据听阈曲线检测结果分为低中频下降组(30例)、高频下降组(30例)和全频下降组(29例)。另选取30例同期体检健康者纳入对照组。记录各组患者血脂水平,检测椎-基底血流动力学指标。结果高频下降组患者大脑椎动脉(VA)的收缩期血流速(Vs)、舒张期血流速(Vd)水平高于对照组(P<0.05)。低中频下降组及高频下降组患者大脑基底动脉(BA)的Vs及高频组大脑BA的Vd水平高于对照组(P<0.05)。观察组患者总胆固醇(TC)、低密度脂蛋白(LDL)水平高于对照组,高密度脂蛋白(HDL)水平低于对照组(P<0.05);两组甘油三酯(TG)水平比较,差异无统计学意义(P>0.05)。观察组不同听阈曲线类型患者TC、LDL-C、HDL-C水平比较,差异有统计学意义(P<0.05)。结论与健康者相比,突发性聋患者的血脂水平异常,且不同听阈曲线类型的突发性聋患者血脂、椎-基底动脉血流动力学均存在异常,血脂异常及血流动力学与突发性聋听阈曲线存在密切联系。
Objective To investigate the characteristics of blood lipids and vertebrobasilar artery hemodynamics in patients with sudden deafness of different hearing threshold curve types.Methods A total of 89 patients with sudden deafness who were examined in the First People’s Hospital of Zhumadian from May 2020 to May 2022 were included in the observation group.According to the hearing threshold curve test results,they were divided into low and medium frequency decline group(30 cases),high frequency decline group(30 cases)and full frequency decline group(29 cases).Another 30 healthy subjects were included in the control group.The blood lipid levels of patients in each group were recorded and the vertebrobasilar hemodynamic indexes were measured.Results The levels of systolic velocity(Vs)and diastolic velocity(Vd)in the cerebral vertebral artery(VA)of patients in the high-frequency group were higher than those in the control group(P<0.05).The levels of Vs of cerebral basilar artery(BA)and Vd of cerebral BA in the low,middle and high frequency group were higher than those in the control group(P<0.05).The levels of total cholesterol(TC)and low density lipoprotein(LDL-C)in the observation group were higher than those in the control group,and the level of high density lipoproteins(HDL)was lower than that in the control group(P<0.05).There was no statistical difference in triglyceride(TG)levels between the two groups(P>0.05).The levels of TC,LDL-C and HDL-C in patients with different hearing threshold curve types in the observation group were statistically different(P<0.05).Conclusion Compared with healthy subjects,patients with sudden deafness have abnormal blood lipid levels,and patients with sudden deafness with different hearing threshold curve types have significant abnormalities in blood lipids and vertebrobasilar artery hemodynamics,the dyslipidemia and hemodynamics are closely related to the hearing threshold curve of sudden deafness.
