基于跨理论模型的健康教育联合治疗性沟通对胃癌患者的干预效果

摘要 目的探究基于跨理论模型的健康教育联合治疗性沟通对胃癌患者的干预效果。方法采用前瞻性研究方式,选取2019年1月至2022年3月于平顶山市第二人民医院确诊胃癌并行胃癌手术的58例患者为研究对象,依据随机数表法分为对照组、观察组,每组29例。两组均接受常规手术与辅助治疗,对照组接受治疗性沟通,观察组在对照组基础上联合以跨理论模型为指导的健康教育。采用状态-特质焦虑调查量表(STAI)、抑郁自评量表(SDS)、癌症应对方式问卷(CCMQ)、胃癌生活质量问卷(QLQ-STO22)以及自拟依从性调查问卷对患者进行评估。结果干预后两组STAI、SDS、QLQ-STO22评分均降低,且观察组低于对照组(P<0.05)。干预后两组面对评分均升高,回避、压抑以及屈服、幻想、发泄评分均降低,且观察组改变程度更明显(P<0.05)。观察组康复锻炼、定期复查、饮食依从性高于对照组(P<0.05)。结论跨理论模型的健康教育联合治疗性沟通可改善胃癌患者心理状态与应对方式,提高治疗依从性与生活质量。 Objective To explore the intervention effect of health education based on cross theoretical model combined with therapeutic communication in patients with gastric cancer.Methods Using the prospective research method,58 patients with gastric cancer diagnosed in the Second People’s Hospital of Pingdingshan from January 2019 to March 2022 and operated on for gastric cancer were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 29 cases in each group.Both groups received routine surgery and adjuvant treatment,the control group received therapeutic communication,and the observation group received health education guided by the cross theoretical model on the basis of the control group.The patients were assessed with state trait anxiety inventory(STAI),self-rating depression scale(SDS),cancer coping modes questionnaire(CCMQ),quality of life questionnaire of stomach 22(QLQ-STO22)and self-made compliance questionnaire.Results After the intervention,the scores of STAI,SDS and QLQ-STO22 in the two groups decreased,and the observation group were lower than those in the control group(P<0.05).After the intervention,the face scores of the two groups increased,and the scores of avoidance,depression,submission,fantasy and venting decreased,and the change degree of the observation group were more obvious(P<0.05).The rehabilitation exercise,regular reexamination and dietary compliance of the observation group were higher than those of the control group(P<0.05).Conclusion Cross theoretical model health education combined with therapeutic communication can improve the psychological state and coping style of patients with gastric cancer after surgery,and improve treatment compliance and quality of life.
