百合育子方对特发性弱精子症及精浆睾酮和氧化应激的影响 被引量:3

Baihe Yuzi Formula in the treatment of idiopathic asthenospermia and its effect on seminal plasma testosterone and oxidative stress

摘要 目的观察基于金水相生理论的百合育子方对肺肾两虚型特发性弱精子症患者精液质量、精子DNA碎片指数(DFI)的影响并探索其作用机制。方法将2020年6月至2021年6月河北省中医院男科门诊的120例肺肾两虚型特发性弱精子症患者随机分为试验组和对照组各60例,分别给予百合育子方和左卡尼汀口服液联合五子衍宗丸治疗3个月,比较两组患者治疗前后精子前向运动率(PR)、活率、PR精子总数、精子DFI以及精浆睾酮(T)和雌二醇(E2)水平、活性氧(ROS)、丙二醛(MDA)和超氧化物歧化酶(SOD)的变化。结果试验组脱失5例,最终55例;对照组脱失7例,最终53例。两组患者的精子PR、活率、PR精子总数、精子DFI均较治疗前改善(P<0.05),且试验组优于对照组(P<0.05);与治疗前比较,治疗后试验组精浆T浓度和总量显著升高(P<0.05),对照组的变化无统计学意义(P>0.05);两组精浆E2的浓度和总量均无显著性差异(P>0.05);与治疗前比较,治疗后试验组、对照组精浆SOD均较治疗前升高(P<0.05),精浆ROS、MDA均较治疗前降低(P<0.05),且试验组优于对照组(P<0.05)。结论百合育子方可显著改善肺肾两虚型特发性弱精子症患者精液质量,降低精子DFI,其作用机制可能与提高精浆T水平,降低氧化应激损伤有关。 Objective To investigate the effects of Baihe Yuzi Formula based on the theory of generation between the metal and water on semen quality and sperm DNA fragmentation index(DFI)of idiopathic asthenospermia patients with lung and kidney deficiency and explore its relevant mechanism.Methods From June 2020 to June 2021,120 idiopathic asthenospermia patients with Lung and kidney Deficiency were randomly divided into the experimental group(n=60)and the control group(n=60).The patients in two groups were treated with Baihe Yuzi Formula and L-carnitine oral liquid combined with Wuzi Yanzong pills for 3 months,respectively.The changes of sperm forward motility rate(PR),sperm viability,Total PR sperm,DNA fragmention index(DFI),testosterone(T),estradiol(E2),reactive oxygen species(ROS),malondialdehyde(MDA)and superoxide dismutase(SOD)in seminal plasma of 2 groups before and after treatment were comparatively analyzed.Results In the experimental group,5 cases were lost,and 55 cases were adopted.In the control group,7 cases were lost,and 53 cases were adopted.The sperm PR,viability,total sperm PR and sperm DFI of two groups were all improved compared with those before treatment(P<0.05),and the experiment group was better than the control group(P<0.05).Compared with those before treatment,the concentration and total amount of seminal plasma T in the experimental group were increased significantly after treatment(P<0.05),but no significant difference was found in the control group(P>0.05);There were no significant differences in the concentration and total amount of E2in seminal plasma between the two groups(P>0.05);Compared with that before treatment,SOD in seminal plasma of the experimental group and the control group was increased(P<0.05),but ROS and MDA in seminal plasma were decreased(P<0.05),and the experimental group was better than the control group(P<0.05).Conclusion Baihe Yuzi Formula can significantly improve sperm motility,reduce DFI,regulate seminal plasma hormone level and reduce oxidative stress damage in idiopathic asthenospermia patients with Lung and kidney Deficiency.
