慢性心力衰竭患者lncRNA LUCAT1表达及其与左心室肥厚、心功能的相关性 被引量:4

Expression of lncRNA LUCAT1 in patients with chronic heart failure and its correlation with left ventricular hypertrophy and cardiac function

摘要 目的探讨慢性心力衰竭(CHF)患者血清长链非编码RNA肺癌相关转录产物1(lncRNA LUCAT1)与心功能及左心室肥厚(LVH)的关系。方法选取2021年2月至2022年3月本院187例CHF患者为研究对象,根据美国纽约心脏病协会(NYHA)心功能分级分为NYHAⅠ~Ⅱ级组(67例)、NYHAⅢ级组(72例)和NYHAⅣ级组(48例);根据是否发生LVH分为非LVH组(119例)和LVH组(68例)。收集CHF患者临床资料;采用实时荧光定量PCR(qRT-PCR)检测血清lncRNA LUCAT1相对表达量;Pearson及Spearman相关分析CHF合并LVH患者血清lncRNA LUCAT1水平与临床资料的相关性;绘制受试者工作特征(ROC)曲线分析血清lncRNA LUCAT1水平对CHF患者发生LVH的预测效能;采用多因素logistic回归分析CHF患者发生LVH的影响因素。结果NYHAⅠ~Ⅱ级组、NYHAⅢ级组、NYHAⅣ级组CHF患者血清lncRNA LUCAT1水平依次降低(P<0.05)。LVH组与非LVH组临床资料中心功能分级、左心室射血分数(LVEF)、左心室质量指数(LVMI)比较差异有统计学意义(P<0.05)。LVH组血清lncRNA LUCAT1水平显著低于非LVH组(P<0.05)。CHF合并LVH患者血清lncRNA LUCAT1水平与LVEF呈正相关(r=0.508,P<0.05),与心功能分级、LVMI呈负相关(r=-0.524、-0.512,P<0.05)。血清lncRNA LUCAT1水平预测CHF患者发生LVH的曲线下面积(AUC)为0.866(95%CI:0.814~0.917),敏感度为79.0%,特异度为83.8%。lncRNA LUCAT1是CHF患者发生LVH的保护因素(P<0.05),心功能分级是CHF患者发生LVH的独立危险因素(P<0.05)。结论lncRNA LUCAT1与CHF患者心功能及LVH间存在相关性,且对CHF患者发生LVH有重要预测效能。 Objective To investigate the correlations between serum long non-coding RNA lung cancer-associated transcript 1(lncRNA LUCAT1)and cardiac function and left ventricular hypertrophy(LVH)in patients with chronic heart failure(CHF).Methods A total of 187 CHF patients from February 2021 to March 2022 were enrolled.According to the New York Heart Association(NYHA)cardiac function classification,they were grouped into NYHA ClassⅠ-Ⅱgroup(n=67),NYHA ClassⅢgroup(n=72)and NYHA ClassⅣgroup(n=48).According to the occurrence of LVH,the patients were grouped into non-LVH group(n=119)and LVH group(n=68).Clinical data of CHF patients were collected.Quantitative real-time PCR(qRT-PCR)was used to assess the relative expression of serum lncRNA LUCAT1.Pearson and Spearman correlation analysis were used to analyze the correlations between serum lncRNA LUCAT1 level and clinical data in patients with CHF and LVH.Receiver operating characteristic(ROC)curve was drawn to analyze the predictive power of serum lncRNA LUCAT1 level on the occurrence of LVH in CHF patients.Multivariate logistic regression was used to analyze the influencing factors of LVH in CHF patients.Results The levels of serum lncRNA LUCAT1 in CHF patients in NYHA ClassⅠ-Ⅱgroup,NYHA ClassⅢgroup,and NYHA ClassⅣgroup were significantly sequentially decreased(P<0.05).There were significant differences between the LVH group and the non-LVH group in clinical data center functional classification,left ventricular ejection fraction(LVEF),and left ventricular mass index(LVMI)(P<0.05).The level of serum lncRNA LUCAT1 in the LVH group was significantly lower than that in the non-LVH group(P<0.05).The level of serum lncRNA LUCAT1 in CHF patients with LVH was positively correlated with LVEF(r=0.508,P<0.05),and negatively correlated with cardiac function grade and LVMI(r=-0.524,-0.512,P<0.05).The area under the curve(AUC)of serum lncRNA LUCAT1 level in predicting LVH in CHF patients was 0.866(95%CI:0.814-0.917),with a sensitivity of 79.0%and a specificity of 83.8%.The lncRNA LUCAT1 was a protective factor for LVH in CHF patients(P<0.05),while cardiac function classification was an independent risk factor for LVH in CHF patients(P<0.05).Conclusion LncRNA LUCAT1 is correlated with cardiac function and LVH in patients with CHF,and has important predictive performance for the occurrence of LVH in patients with CHF.
