全程陪伴式导乐护理对顺产初产妇镇痛效果及满意度的影响

Effect of Whole Course Accompanying Doula Nursing on Analgesic Effect and Satisfaction of Primiparous Women With Natural Labor

目的分析全程陪伴式导乐护理对顺产初产妇镇痛效果及满意度的影响。方法选取2021年2月—2022年2月厦门大学附属妇女儿童医院收治的180例顺产初产妇作为研究对象,将其中的90例接受全程陪伴式导乐护理的产妇设为研究组,另外90例常规护理管理的产妇设为参考组,对两组产妇疼痛程度、总产程时间、产妇不良心理评分改善情况、分娩结局、产妇满意度进行对比分析。结果研究组初产妇总满意率为96.67%,参考组初产妇总满意率为83.33%,研究组初产妇总满意率明显高于参考组,差异有统计学意义(P<0.05);研究组产后出血、新生儿窒息等不良妊娠结局率明显低于参考组,差异有统计学意义(P<0.05),不良心理改善情况明显优于参考组,差异有统计学意义(P<0.05),新生儿Apgar评分、动脉血气pH值与参考组相比差异无统计学意义(P>0.05)。同时,研究组产妇总产程时间均明显短于参考组,差异有统计学意义(P<0.05)。结论全程陪伴式导乐护理能有效提高镇痛效果和产妇满意率,并有助于缩短产妇的产程,促进产妇不良心理改善,降低不良妊娠结局率。

Objective To analyze the effect of whole course accompanying doula nursing on analgesic effect and satisfaction of primiparas with natural labor.Methods From February 2021 to February 2022,a total of 180 primiparas with vaginal delivery admitted to Women and Children’s Hospital,School of Medicine,Xiamen University were selected as the research objects.Among them,a total of 90 primiparas who received whole-course accompanying doula care were set as the research group,and the other 90 primiparas who received routine care management were set as the reference group.The pain degree,total labor time,improvement of maternal adverse psychological score,delivery outcome and maternal satisfaction were compared and analyzed between the two groups.Results The total satisfaction rate of primiparas in the study group was 96.67%,and that in the reference group was 83.33%.The total satisfaction rate of primiparas in the study group was significantly higher than that in the reference group,and the difference was statistically significant(P<0.05).The rate of adverse pregnancy outcomes such as postpartum hemorrhage and neonatal asphyxia in the study group was significantly lower than that in the reference group,and the difference was statistically significant(P<0.05).The adverse psychological improvement was significantly better than that in the reference group,and the difference was statistically significant(P<0.05).There was no significant difference in neonatal Apgar score and arterial blood gas pH between the two groups(P>0.05).Meanwhile,the total labor time of the study group was significantly shorter than that of the reference group,and the difference was statistically significant(P<0.05).Conclusion Doula nursing can effectively improve the analgesic effect and the satisfaction rate of parturient women,and help shorten the labor process of parturient women,promote the improvement of maternal adverse psychology and reduce the rate of adverse pregnancy outcomes.
