经鼻持续气道正压通气联合肺泡表面活性物质治疗新生儿呼吸窘迫综合征的效果

Effect of nasal continuous positive airway pressure combined with Pulmonary Surfactant in the treatment of neonatal respiratory distress syndrome

目的分析将经鼻持续气道正压通气(CPAP)联合肺泡表面活性物质用于新生儿呼吸窘迫综合征(NRDS)治疗中效果。方法选取邳州市人民医院2020年1月至2021年8月收治的56例NRDS患儿,以随机数字表法分为两组,每组各28例。对照组采用常规治疗和CPAP,观察组在其基础上加用肺泡表面活性物质,比较两组治疗效果。结果观察组总有效率为92.86%,较对照组的67.86%更高(P<0.05)。观察组血清重组人巨噬细胞移动抑制因子-1、人高迁移率族蛋白B1水平较对照组更低(P<0.05)。观察组pH值、动脉血氧分压、氧合指数水平高于对照组,动脉血二氧化碳分压较对照组更低(P<0.05)。结论将CPAP联合肺泡表面活性物质用于NRDS治疗中可改善患儿血清学、血气指标,提升临床疗效。

Objective To analyze the effect of nasal continuous positive airway pressure(CPAP)combined with Pulmonary Surfactant in the treatment of neonatal respiratory distress syndrome(NRDS).Methods A total of 56 children with NRDS admitted to Pizhou People’s Hospital from January 2020 to August 2021 were selected and divided into two groups by random number table method,with 28 cases in each group.The control group was given routine treatment and CPAP,the observation group added Pulmonary Surfactant,and the treatment effect of two groups was compared.Results The total effective rate of the observation group was 92.86%,higher than 67.86%of the control group(P<0.05).The levels of macrophage migration inhibitory factor-1 and high mobility group protein box-1 in the observation group were lower than those in the control group(P<0.05).The pH value,arterial partial pressure of oxygen and oxygenation index in the observation group were higher than those in the control group,arterial partial pressure of carbon dioxide was lower than that in the control group(P<0.05).Conclusion The use of CPAP combined with Pulmonary Surfactant in the treatment of NRDS can improve the serological and blood gas indexes of children,enhance the clinical efficacy.
