摘要
目的探讨基于改良早期预警评分(modified early warning score,MEWS)的转运管理模式在急危重症患者院内转运中的应用效果。方法选取2020年1月至2022年1月抚州市第一人民医院收治的急危重症患者116例,按照随机数字表法将其分为对照组和观察组,每组各58例。对照组患者采取常规转运方法,观察组患者采取基于MEWS的转运管理模式。比较两组患者的院内转运时间、不良事件发生情况和满意度及护理人员的批判性思维能力。结果观察组患者转运至检查室、手术室、介入室、重症监护室和普通病房的时间均显著短于对照组(P<0.05);观察组患者的不良事件发生率显著低于对照组(3.45%vs.13.79%,χ^(2)=3.940,P=0.047);干预后,两组护理人员的中文版批判性思维能力测量表(Chinese version of critical thinking disposition inventory,CTDI-CV)评分均显著高于本组干预前,且观察组护理人员的CTDI-CV各维度评分均显著高于对照组(P<0.05);观察组患者对转运方式、安全性、操作技术和服务态度的满意度评分均显著高于对照组(P<0.05)。结论基于MEWS的转运管理模式能够缩短急危重症患者的院内转运时间,降低不良事件发生率,有助于增强护理人员的批判性思维能力,从而提升患者满意度。
Objective To explore the application effect of the transport management based on modified early warning score(MEWS)in the hospital transport of critical patients.Methods A total of 116 critical patients admitted to Fuzhou First People’s Hospital from January 2020 to January 2022 were selected and divided into control group and observation group according to random number table method,with 58 cases in each group.Patients in control group received conventional transport methods,and patients in observation group received transport management based on MEWS.Hospital transport time,adverse event occurrence and satisfaction of patient,and critical thinking ability of nursing staff were compared between the two groups.Results The transport time to examination room,operating room,intervention room,intensive care unit and general ward in observation group was significantly shorter than that in control group(P<0.05).The incidence of adverse events was significantly lower in observation group than in control group(3.45%vs.13.79%,χ^(2)=3.940,P=0.047).After intervention,the scores of the Chinese version of critical thinking disposition inventory(CTDI-CV)in both groups were significantly higher than those before intervention,and CTDI-CV scores of observation group were significantly higher than those of control group(P<0.05).The satisfaction scores of transport mode,safety,operation technique and service attitude in observation group were significantly higher than those in control group(P<0.05).Conclusion The transport management based on MEWS can shorten the hospital transport time of critical patients,reduce the incidence of adverse events,and help to enhance the critical thinking ability of nursing staff,thus improving patient satisfaction.
