术前超声联合MRI对乳腺癌患者腋窝淋巴结状态的评价效能

Analysis of preoperative ultrasound combined with MRI in the evaluation of axillary lymph node status in breast cancer patients

目的研究术前超声联合磁共振成像(MRI)对乳腺癌患者腋窝淋巴结状态的评价效能,为临床乳腺癌患者ALNM的及时诊治提供参考依据。方法选取2021年2月至2022年1月以“乳腺肿物”收住院的42例患者作为研究对象。受试者术前均接受超声及MRI检查,以术后淋巴结病理结果为依据,运用诊断性试验的研究方法,对比分析超声、MRI以及两者联合应用评估腋窝淋巴结转移(ALNM)的准确性等诊断效能。结果以病理结果为金标准,超声诊断ALNM的灵敏度、特异度以及准确度分别为58.33%(7/12)、70.00%(20/30)、66.67%(28/42)。以病理结果为金标准,MRI诊断ALNM的灵敏度、特异度以及准确度分别为75.00%(9/12)、66.67%(20/30)、69.05%(29/42)。以病理结果为金标准,超声或MRI阳性诊断ALNM的灵敏度、特异度以及准确度分别为75.00%(9/12)、53.33%(16/30)、59.52%(25/42)。以病理结果为金标准,超声与MRI同时阳性诊断ALNM的灵敏度、特异度以及准确度分别为58.33%(7/12)、83.33%(25/30)、76.19%(32/42)。结论术前超声联合MRI对乳腺癌患者ALNM的诊断效能较佳,值得临床推广应用。

Objective To study the efficacy of preoperative ultrasound combined with magnetic resonance imaging(MRI)in the evaluation of axillary lymph node metastasis(ALNM)in breast cancer(BC)patients,aiming to provide reference for the timely diagnosis and treatment of ALNM.Methods From February 2021 to June 2022,totally 42 patients with breast mass admitted to our hospital were enrolled as the research object,the subjects underwent ultrasound and MRI examinations before surgery.Based on the pathological results of postoperative lymph nodes,the diagnostic test accuracy studies were performed to assess the accuracy of ultrasound,MRI,and the combination of the two in evaluating ALNM.and other diagnostic performance were recorded.Results With pathological results as the gold standard,the sensitivity,specificity,and accuracy of ultrasound for ALNM diagnosis:58.33%(7/12),70.00%(20/30)and 66.67%(28/42);of MRI:75.00%(9/12),66.67%(20/30)and 69.05%(29/42);of ultrasound or MRI when either one’s results for ALNM diagnosis was positive:75.00%(9/12),53.33%(16/30),and 59.52%(25/42);of the combination of ultrasound and MRI application when the both results for ALNM diagnosis was positive:58.33%(7/12),83.33%(25/30),76.19%(32/42).Conclusion Preoperative ultrasound combined with MRI has a better diagnostic efficacy for ALNM in BC patients,which is worthy of clinical application.
