儿童期创伤对健康青年脑结构和功能改变的研究 被引量:1

The changes of brain structure and function in healthy youths caused by childhood trauma

摘要 目的本研究通过儿童期创伤问卷(childhood trauma questionnaire,CTQ)和磁共振扫描,采集健康青年人心理学及影像学数据,探讨儿童期创伤经历对大脑脑区结构改变和脑白质纤维束的影响。方法本研究共纳入86例受试者,采集所有受试者大脑磁共振图像,包括结构像和弥散张量成像(diffusion tensor imaging,DTI),根据CTQ评分,将受试者分为有童年创伤经历20例(儿童期创伤组),没有童年创伤66例(正常对照组),使用Freesurfer软件对脑区进行分割,统计分析儿童期创伤组和正常对照组脑区结构体积改变,采用FSL软件分析脑白质连接变化。结果与正常对照组相比,儿童期创伤组左侧杏仁核体积、海马体积明显低于正常组(P<0.05);额中回、右侧额枕下束、下纵束、扣带回和楔前叶周围双侧丘脑前辐射分数各向异性(FA)下降(P<0.05)。结论童年创伤影响大脑结构和功能,可能会改变杏仁核及眼窝前额皮质体积及降低某些脑区白质连接。 Objective To investigate the effects of childhood trauma on the brain structure and alba fiber bundle in healthy youths through Childhood Trauma Questionnaire(CTQ)and magnetic resonance scanning.Methods A total of 86 subjects were enrolled in the study,and the magnetic resonance Imaging(MRI)images of all the subjects were collected,including structural imaging and diffusion tensor imaging(DTI).According to CTQ score,these subjects were divided into two groups:observation group(with childhood trauma,n=20)and control group(without childhood trauma,n=66).The Freesurfer software was used to segment the brain regions,and the structural volume changes of the brain regions in both groups were statistically analyzed.Moreover FSL software was used to analyze the changes of white matter connections.Results The volumes of amygdala and hippocampus in observation group were significantly lower than those in control group(P<0.05).As compared with that in control group,the radiation fraction anisotropy(FA)in middle frontal gyrus,right-side forehead pillow inferior cords,inferior longitudinal fasciculus,cingulate gyrus and hippocampus in observation group was significantly decreased(P<0.05).Conclusion Childhood trauma affects the structure and function of the brain of patients with childhood trauma,which may change the volume of the amygdala and orbital prefrontal cortex and reduce the white matter connections in certain brain areas.
