Copeptin在肥胖儿童中升高并与胰岛素抵抗密切相关

Copeptin is significantly elevated in obese children and closely related to insulin resistance

目的 分析肥胖儿童血清中Copeptin的变化及其与胰岛素抵抗之间的关系。方法 选取2018年9月至2019年9月就诊于西安交通大学第二附属医院儿童内分泌门诊的肥胖儿童80名作为肥胖组,正常体重儿童40名作为对照组。测量所有儿童的腰围、腹围、体重、血压等指标,检测空腹血糖、血脂及胰岛素,并使用ELISA测定Copeptin的水平,比较两组儿童上述指标。结果 与对照组相比,Copeptin在肥胖组儿童血液中显著升高(t=5.120,P<0.05);肥胖组中胰岛素抵抗患儿血清中Copeptin与对照组比较也显著升高(t=3.894,P<0.05)。在偏相关分析中矫正年龄、性别和SDS-BMI后,发现胰岛素、HOMA-IR、DBP仍与Copeptin呈显著性正相关(r值分别为0.195、0.189、0.227,P<0.05);多元线性回归分析显示,除SDS-BMI、腰围外,Copeptin是HOMA-IR指数相关的独立危险因素(β=0.235,P<0.05)。结论 Copeptin与肥胖儿童胰岛素抵抗密切相关,Copeptin可以用作预测胰岛素抵抗的可靠生物标志物。

Objective This study aims to analyze the changes of Copeptin in the serum and the relationship between Copeptin and insulin resistance in obese children.Methods A total of 80 obese children admitted to the children Endocrinology Clinic of the Second Affiliated Hospital of Xi′an Jiaotong University from September 2018 to September 2019 were selected as the obesity group, and 40 children with normal weight were selected as the control group.The waist circumference, abdominal circumference, weight, blood pressure and other indicators of all subjects were measured, and fasting blood glucose, blood lipid and insulin were detected, and the levels of Copeptin were determined by ELISA,and the above indexes were compared between the two groups.Results Compared with the control group, the level of Copeptin was significantly increased in the obese group(t=5.120,P<0.05),and the serum Copeptin in obese children with insulin resistance was significantly higher than that in the control group(t=3.894,P<0.05).After adjusting for age, gender, and SDS-BMI in partial correlation analysis, insulin, HOMA-IR,DBP were still significantly positively correlated with Copeptin(r=0.195,0.189 and 0.227,respectively, P<0.05).Multiple linear regression analysis showed that Copeptin was an independent risk factor associated with HOMA-IR index except for SDS-BMI and waist circumference(β=0.235,P<0.05).Conclusion Copeptin is closely related to insulin resistance in obese children, Copeptin can be used as a reliable biomarker to predict insulin resistance.
