运脾强生方穴位贴敷改善腹膜透析患者疲乏

Acupoint Application of Yunpi Qiangsheng Recipe in Improvement ofFatigue of Peritoneal Dialysis Patients

目的:观察在腹膜透析(peritoneal dialysis,PD)基础上加用运脾强生方穴位贴敷改善疲乏的疗效。方法:选取行规律PD患者42例,分为观察组20例,对照组22例。两组均给予常规PD及基础治疗,观察组加用运脾强生方穴位贴敷,观察2个月,对比两组疲乏量表、中医症状积分量表、生存质量量表、实验室指标等。结果:治疗后,观察组患者疲劳量表总分较治疗前下降,且下降幅度优于对照组(P<0.01)。两组患者疲劳程度(因子1)、环境敏感性(因子2)平均分较治疗前下降,因子2下降幅度高于对照组(P<0.05)。治疗后观察组中医症状缓解有效率20%,对照组有效率14%,两组比较,差异无统计学意义(P>0.05),其中观察组气短懒言、面肢水肿等症状改善情况优于对照组(P<0.05)。治疗后观察组在患者满意度、生理机能、肾脏病的压力评分方面优于对照组(P<0.05)。治疗后观察组血清白蛋白、前白蛋白、血红蛋白较前升高,前白蛋白水平高于对照组(P<0.05);尿素氮、全段甲状旁腺激素、超敏C-反应蛋白较前降低,其中超敏C-反应蛋白显著下降(P<0.01)。结论:运脾强生方穴位贴敷结合基础治疗,可显著减轻PD患者疲乏程度,改善中医症状、营养状况、炎症状态,提高生存质量,为透析患者的临床康复提供了治疗途径。

Objective:To observe the curative effect of Yunpi Qiangsheng Recipe acupoint application on the basis of peritoneal dialysis(PD)in improving fatigue.Methods:A total of 42 regular PD patients were selected and divided into an observation group of 20 cases and a control group of 22 cases.Both groups were given conventional PD and basic treatment,and the observation group was additionally given Yunpi Qiangsheng Recipe for acupoint application.After 2 months of observation,the fatigue scale,TCM symptom score scale,quality of life scale,and laboratory indicators were compared between the two groups.Results:After treatment,the total average score of the fatigue scale in the observation group was lower than that before treatment,and the decline rate was better than that in the control group(P<0.01).The average scores of fatigue(factor 1)and environmental sensitivity(factor 2)in the two groups were lower than those before treatment,and the decline range of factor 2 was higher than that in the control group(P<0.05).After treatment,the effective rate of TCM symptom relief in the observation group was 20%,and that in the control group was 14%.There was no statistically significant difference between the two groups(P>0.05).Control group(P<0.05).Among them,the improvement of symptoms such as shortness of breath,laziness to speak,and edema of the face and extremities in the observation group was better than that in the control group(P<0.05).After treatment,the observation group was better than the control group in terms of patient satisfaction,physiological function,and kidney disease stress score(P<0.05).After treatment,serum albumin,prealbumin,and hemoglobin in the observation group were higher than before,and the level of prealbumin was higher than that in the control group(P<0.05).Blood urea,intact parathyroid hormone,and high-sensitivity C-reactive protein were lower than before,among them,high-sensitivity C-reactive protein decreased significantly(P<0.01).Conclusion:Yunpi Qiangsheng Recipe acupoint application combined with basic treatment can significantly reduce the fatigue degree of PD patients,improve TCM symptoms,nutritional status,inflammatory state,improve the quality of life,and provide a therapeutic approach for the clinical rehabilitation of dialysis patients.
