摘要
[目的]分析胃底腺多发性息肉的临床特征,探索胃底腺多发性息肉发病机制。[方法]收集胃底腺多发性息肉>10个的患者132例,排除家族性腺瘤性息肉病、幼年性息肉病、Peutz-Jgehers综合征及胃增生性息肉。分析患者的基本信息、饮食习惯、合并疾病及用药情况;对其中37例患者的组织采用免疫组织化学染色检测COX-2、胃泌素、细胞增殖抗原标记物Ki-67蛋白的表达情况。[结果]132例中89%为女性、中年居多,息肉数目最多为≥100个。37例胃底腺多发性息肉组织中COX-2表达阳性率为100%、其中强表达占51.4%(19例),胃泌素表达阳性率为16.2%(6例),Ki-67表达阳性率为13.5%(5例)。[结论]胃底腺多发性息肉以中年女性为主,息肉组织中COX-2高表达,提示COX-2抑制剂可在预防胃底腺多发性息肉反复增生中具有一定作用。
[Objective]To analyze the clinical features of multiple gastric fundus polyps and to explore the pathogenesis of gastric fundus gland polyps.[Methods]Patients with more than 10 fundus glandular polyps were collected, and familial adenomatous polyposis, juvenile polyposis, Peutz-Jgehers syndrome and gastric hyperplastic polyps were excluded.The general characters, eating habits, combined diseases and medication in the patients with multiple polyps of gastric fundus gland were analyzed. Immunohistochemical staining was used to detect the expression of COX-2,gastrin and cell proliferation antigen marker Ki-67 protein in 37 cases of patients with multiple gastric fundus gland polyps.[Results]A total of 132 patients with multiple polyps of gastric fundus gland were collected.The characteristics were that 89% of them were female, most of them were middle-aged, and the number of polyps was more than 100.The positive rates of COX-2,gastrin and Ki-67 in 37 cases of multiple polyps of gastric fundus gland were 100%,16.2% and 13.5%,respectively.[Conclusion]Multiple polyps of fundus glands are mainly in middle-aged women, and COX-2 is highly expressed in polyps, which suggests that COX-2 inhibitors may play a role in preventing recurrent hyperplasia of multiple polyps in fundus glands.
