双侧3种病理类型肾癌的手术治疗并文献复习

Surgical treatment of three pathological types of bilateral renal cell carcinoma and review of literature

目的探讨分析双侧肾脏不同病理类型的恶性肿瘤治疗方案。方法回顾性分析2021年7月23日湖北医药学院附属太和医院泌尿外科收治的1例双侧肾脏3种不同病理类型的肾恶性肿瘤患者的临床资料并对此种病例进行文献复习。结果患者于麻醉下分期进行机器人辅助左侧肾部分切除术及机器人辅助右侧肾部分切除术,两次手术均顺利完成,术后病理学检查提示:左侧透明细胞性肾细胞癌、乳头状肾细胞癌Ⅱ型;右侧透明细胞性肾细胞癌、低度恶性潜能的多房囊性肾肿瘤。结论对于双侧不同病理类型的肾细胞癌,采用机器人辅助分期手术的治疗方案在恶性肿瘤的完整切除、肾单位的保留及术后患者的恢复方面均具有良好的效果,此方案可作为双侧不同病理类型的肾细胞癌的优选方案。

【Objective】To explore and analyze the treatment of bilateral renal malignant tumors with different pathological types.【Methods】The clinical data of 1 patient with bilateral renal malignant tumor with 3 different pathological types admitted to Department of Urology,Taihe Hospital,Shiyan city on July 23,2021 were retrospectively analyzed and the literatures were reviewed.【Results】The patient underwent robot-assisted partial nephrectomy on the left side and robot-assisted partial nephrectomy on the right side in stages under anesthesia,and both operations were successfully completed.Postoperative pathological examination suggested left clear cell renal cell carcinoma and papillary renal cell carcinoma type II.This is a clear cell renal cell carcinoma at the right.This is a multilocular cystic renal tumor with low malignant potential.【Conclusion】For bilateral renal cell carcinoma with different pathological types,the treatment plan of robot-assisted staging surgery has a good effect on the complete resection of malignant tumor,the preservation of nephron and the postoperative recovery of patients,and this plan can be used as the optimal plan for bilateral renal cell carcinoma with different pathological types.
