非良性部分囊性甲状腺结节的超声图像特征

Ultrasound Characteristics of Nonbenign Partially Cystic Thyroid Nodules

目的分析甲状腺部分囊性结节中非良性结节超声图像特征。方法回顾性分析64例经术后病理证实的非良性部分囊性甲状腺结节的超声图像特征,计算病例中出现这些图像特征的比例。结果64例非良性部分囊性甲状腺结节中,乳头状癌42例,非良性滤泡性肿瘤22例。1)部分囊性甲状腺乳头状癌超声图像特征:结节最大径(23.26±12.31)mm,多数病例为单发结节(69.05%),偏心锐角型(57.14%),内部以实性成分为主(73.81%),结节边界不清(61.90%),形态不规则(66.66%),低回声(61.90%),有微钙化(80.95%),血流不丰富(88.10%)。2)非良性部分囊性甲状腺滤泡肿瘤超声图像特征:结节最大径(39.91±8.93)mm,多数病例为单发结节(81.82%),子囊型(90.90%),子囊多为少囊(70%)和小囊(65%),内部以实性成分为主(95.45%),结节边界不清(54.55%),形态规则(86.36%),等回声(54.55%),无微钙化(86.36%),血流丰富(86.36%)。结论甲状腺非良性部分囊性结节的超声图像特征有助于临床诊断。

Objective To analyze the ultrasonographic characteristics of nonbenign partially cystic thyroid nodules.Methods Ultrasonographic features of 64 cases of nonbenign partially cystic thyroid nodules confirmed by postoperative pathology were retrospectively analyzed,and the proportion of cases with these image features were calculated.Results Among the 64 cases,42 had papillary carcinoma,and 22 had nonbenign follicular neoplasm.For the ultrasonographic features of partially cystic papillary thyroid carcinoma,the maximum diameter of the nodules was(23.26±12.31)mm,and most cases showed solitary nodule(69.05%),eccentric configuration with an acute angle(57.14%),interior solid components(73.81%),indistinct borders(61.90%),irregular shape(66.66%),hypoechoic sign(61.90%),microcalcification(80.95%),and poor blood flow(88.10%).For the ultrasonographic features of nonbenign partially cystic papillary thyroid carcinoma,the maximum diameter of the nodules was(39.91±8.93)mm,and most cases showed solitary nodule(81.82%),ascus type(90.90%),oligocystic type(70%),microcystic type(65%),interior solid components(95.45%),indistinct borders(54.55%),regular shape(86.36%),isoechoic sign(54.55%),non-microcalcification(86.36%),and abundant blood flow(86.36%).Conclusion Ultrasonographic characteristics can contribute to the diagnosis of nonbenign partially cystic thyroid nodules.
