腹腔镜下卵巢子宫内膜异位囊肿剥除术后复发率及其影响因素分析

Recurrence rate and influencing factors of ovarian endometriosis cyst after endoscopic cystectomy

目的分析腹腔镜下卵巢子宫内膜异位囊肿剥除术后复发率及其影响因素。方法回顾性分析2019年5月至2022年2月于天长市中医院行腹腔镜下卵巢子宫内膜异位囊肿(OEC)剥除术的82例OEC患者的临床资料。记录患者术后OEC复发率,比较不同复发情况患者的临床资料,并分析相关因素与术后OEC复发情况的关系。结果82例患者术后复发率为23.17%。根据复发情况将患者分为复发组(n=19)和未复发组(n=63),两组年龄、吸烟、盆腔手术史、孕次、产次、囊肿直径、是否黏连、r-AFS分期、术后用药及癌抗原125(CA125)水平差异具有统计学意义(P<0.05);多因素分析显示,年龄、盆腔手术史、是否黏连、r-AFS分期及术后用药均为腹腔镜下OEC剥除术后复发的独立影响因素(P<0.05)。结论腹腔镜下OEC剥除术后的复发率较高,术后未用药治疗、年龄<35岁、有盆腔手术史、有黏连及r-AFS分期Ⅲ~Ⅳ期为术后OEC复发的独立危险因素。

Objective To analyze the recurrence rate and influencing factors of ovarian endometriosis cysts(OEC)after endoscopic cystectomy.Methods A retrospective analysis was performed on the clinical data of 82 patients with OEC undergoing endoscopic cystectomy in Tianchang Hospital of Traditional Chinese Medicine between May 2019 and February 2022.The postoperative recurrence rate of OEC was recorded.The clinical data in patients with different recurrence conditions were compared,and relationship between the related factors and postoperative recurrence of OEC was analyzed.Results In the 82 patients,postoperative recurrence rate of OEC was 23.17%.According to presence or absence of recurrence,patients were divided into recurrence group(n=19)and non-recurrence group(n=63).There were significant differences in age,smoking history,history of pelvic surgery,gravidity,parity,cysts diameter,adhesion,r-AFS staging,postoperative medication and CA125 level between the two groups(P<0.05).Multivariate analysis showed that age,history of pelvic surgery,adhesion,r-AFS staging and postoperative medication were all independent influencing factors of OEC recurrence after endoscopic cystectomy(P<0.05).Conclusions The recurrence rate of OEC is high after endoscopic cystectomy.Non-medication after surgery,age<35 years,history of pelvic surgery,adhesion and r-AFS staging at stagesⅢ-Ⅳare independent risk factors of OEC recurrence.
