2017-2020年河南省安阳市慢性病死亡趋势分析

Trend analysis of chronic disease death in Anyang city, Henan province from 2017 to 2020

目的 了解2017-2020年安阳市慢性病死亡情况及死因变化趋势。方法 利用中国死因监测系统收集2017-2020年居民死因监测数据,依据国际疾病分类第10版(ICD-10)对疾病进行分类,应用Excel 2003和SPSS 21.0计算粗死亡率、标化死亡率、年龄别死亡率、死因别死亡率,采用χ^(2)检验比较性别、年龄别死亡率差异,描述安阳市居民死亡情况;采用趋势χ^(2)检验慢性病死亡变化趋势。结果 2017-2020年安阳市慢性病平均粗死亡率为481.39/10万,标化死亡率为412.89/10万,2017-2020年慢性病死亡率呈上升趋势(χ^(2)_(趋势)=216.169,P<0.001);前5位死因是循环系统疾病、肿瘤、呼吸系统疾病、内分泌及营养和代谢疾病、消化系统疾病。2017-2020年不同性别及总慢性病死亡率随年龄增加呈上升趋势(P<0.001)。循环系统疾病、肿瘤、内分泌及营养和代谢疾病死亡率呈逐年上升趋势(χ^(2)_(趋势)=229.780、48.503、7.891,P<0.05),呼吸系统疾病死亡率呈逐年下降趋势(χ^(2)_(趋势)=38.396,P<0.001)。结论 安阳市慢性病总死亡趋势逐年上升,且随年龄增大,死亡率上升。循环系统疾病和肿瘤死亡率逐年上升,是导致慢性病死亡率上升的主要原因,应重点防治。

Objective To understand the change trend of the mortality rate and death causes of chronic disease in Anyang residents from 2017-2020.Methods The death cause monitoring data in the residents from 2017 to 2020 were extracted from the China Death Cause Monitoring System.Diseases are classified according to the 10th edition of the International Classification of Diseases (ICD-10).Excel 2003 and SPSS 21.0 were used to calculate the crude mortality rate,standardized mortality rate,age-specific mortality rate and cause-specific mortality rate.χ^(2)test was used to compare the sex-specific and age-specific mortality rates,and the mortality of residents was described in Anyang city.χ^(2)_(trend) test was conducted to compare the change trend of death.Results The crude mortality rate and the standardized mortality rate in Anyang residents were 481.39/100 000 and 412.89/100 000,respectively,from 2017-2020.From 2017 to 2020,the mortality rate of chronic diseases showed an upward trend (χ^(2)_(trend)=216.169,P<0.001).The top five causes of death from chronic diseases were circulatory system diseases,tumor,respiratory system diseases,endocrine,nutritional and metabolic diseases and digestive system diseases.From 2017 to2020,chronic diseases showed a rising trend in males,females,and both with the increase of age (P<0.001).The mortality rate of circulatory diseases,tumor and endocrine,nutritional and metabolic diseases increased year by year (χ^(2)_(trend)=229.780,48.503,7.891,P<0.05);the mortality rate of respiratory diseases showed a decreasing trend year by year (χ^(2)rend=38.396,P<0.001).Conclusion The overall mortality trend of chronic diseases in Anyang city is increasing year by year,and the mortality rate increases with age;the mortality rate of circulatory system diseases and tumors increases year by year,which is the main cause of the mortality rate of chronic diseases,and the focus of prevention and treatment.
