内镜止血在急性非静脉曲张性上消化道出血治疗中的临床价值

Clinical Value of Endoscopic Hemostasis in Treatment of Acute Non-Variceal Upper Gastrointestinal Bleeding

目的:分析内镜止血在急性非静脉曲张性上消化道出血治疗中的临床价值。方法:选取2020年2月—2022年1月甘肃医学院附属医院收治的70例急性非静脉曲张性上消化道出血患者为研究对象,根据随机抽签分为两组,各35例。对照组采取常规内科止血治疗,观察组采取内镜止血治疗。对比两组治疗效果、治疗指标、优良率及凝血指标。结果:观察组止血成功率高于对照组,再出血率低于对照组,差异有统计学意义(P<0.05)。观察组止血时间、肠鸣音恢复时间及住院时间短于对照组,输血量少于对照组,差异有统计学意义(P=0.000)。观察组优良率高于对照组,差异有统计学意义(P=0.039)。治疗后,观察组凝血酶原时间、凝血酶时间均低于对照组,差异有统计学意义(P=0.000)。结论:急性非静脉曲张性上消化道出血患者采取内镜止血,效果显著,预后较好。

Objective:To analyze the clinical value of endoscopic hemostasis in the treatment of acute non-variceal upper gastrointestinal bleeding.Methods:A total of 70 patients with acute non-variceal upper gastrointestinal bleeding admitted to the Affiliated Hospital of Gansu Medical College from February 2020 to January 2022 were selected as the study subjects.They were divided into two groups according to random lottery method,with 35 patients in each group.The control group received routine medical hemostasis treatment,and the observation group received endoscopic hemostasis treatment.The treatment effect,treatment index,excellent and good rate and coagulation index of the two groups were compared.Results:The success rate of hemostasis in the observation group was higher than that in the control group,and the rebleeding rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The hemostasis time,bowel sound recovery time and length of hospital stay in the observation group were shorter than those in the control group,and the blood transfusion volume in the observation group was less than that in the control group,and the difference was statistically significant(P=0.000).The excellent and good rate in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.039).After treatment,the prothrombin time and thrombin time in the observation group were lower than those in the control group,and the differences were statistically significant(P=0.000).Conclusion:Endoscopic hemostasis in patients with acute non-variceal upper gastrointestinal bleeding has significant effect and good prognosis.
