尿糖与尿微量白蛋白联合检测诊断糖尿病早期肾损伤的应用价值 被引量:2

摘要 目的:分析尿糖与尿微量白蛋白联合检测对诊断糖尿病早期肾损伤的应用价值。方法:选取2019年1月—2021年12月孝感市孝南区妇幼保健院收治的84例糖尿病患者为研究对象,根据疾病的类型分为糖尿病肾损伤组与糖尿病非肾损伤组,另选取同期36例健康体检者为健康体检组。空腹收集三组研究对象尿液,检测尿微量白蛋白和尿糖。比较三组尿微量白蛋白和尿糖检测阳性率、尿微量白蛋白和尿糖水平,以及糖尿病肾损伤组尿微量白蛋白和尿糖单一诊断和联合诊断的准确率。结果:糖尿病非肾损伤组、健康体检组尿微量白蛋白和尿糖检测阳性率低于糖尿病肾损伤组,差异有统计学意义(P<0.05);健康体检组尿微量白蛋白和尿糖检测阳性率低于糖尿病非肾损伤组,差异有统计学意义(P<0.05)。糖尿病非肾损伤组、健康体检组尿微量白蛋白和尿糖水平低于糖尿病肾损伤组,差异有统计学意义(P<0.05);健康体检组尿微量白蛋白和尿糖水平低于糖尿病非肾损伤组,差异有统计学意义(P<0.05)。尿微量白蛋白和尿糖联合诊断准确率高于尿微量白蛋白、尿糖单独检测,差异有统计学意义(P<0.05)。结论:对糖尿病患者使用尿糖联合尿微量白蛋白检测可以判断是否存在早期肾损伤,诊断结果显著,可以为糖尿病肾病早期及时进行干预提供有效依据。 Objective:To analyze the application value of combined detection of urine glucose and urine microalbumin on the diagnosis of early diabetic renal injury.Methods:A total of 84 patients with diabetes admitted to Xiaonan District Maternal and Child Health Hospital of Xiaogan City from January 2019 to December 2021 were selected as the study subjects.According to the type of disease,they were divided into diabetic renal injury group and diabetic non-renal injury group.In addition,36 health examination subjects in the same period were selected as the health examination group.Urine was collected at fasting condition to detect urine microalbumin and urine glucose.The positive rate of urine microalbumin and urine glucose,the level of urine microalbumin and urine glucose in the three groups,and the accuracy of single diagnosis and combined diagnosis of urine microalbumin and urine glucose in the diabetic renal injury group were compared.Results:The positive rates of urine microalbumin and urine glucose detection in the diabetic non-renal injury group and the health examination group were lower than those in the diabetic renal injury group,and the difference was statistically significant(P<0.05).The positive rates of urine microalbumin and urine glucose detection in the health examination group were lower than those in the diabetic non-renal injury group,and the difference was statistically significant(P<0.05).The urine microalbumin and urine glucose levels in the diabetic non-renal injury group and the health examination group were lower than those in the diabetic renal injury group,and the difference was statistically significant(P<0.05).The urine microalbumin and urine glucose levels in the health examination group were lower than those in the diabetic non-renal injury group,and the difference was statistically significant(P<0.05).The accuracy of combined diagnosis of urine microalbumin and urine glucose was higher than that of single diagnosis of urine microalbumin and urine glucose alone,and the difference was statistically significant(P<0.05).Conclusion:The detection of urine glucose and urine microalbumin in diabetic patients can determine whether there is early renal injury.The diagnosis results is significant and can provide effective evidence for early intervention of diabetic nephropathy.
