Research Progress of Kidney Transplantation From Hepatitis B Surface Antigen Positive Donor to Hepatitis B Surface Antigen Negative Recipient

肾移植已成为肾病终末期患者的首选治疗方法,其进一步的推广与发展仍面临部分问题,供体不足仍是限制肾移植手术开展的重要因素之一。乙肝感染者的肾脏是重要的供体来源,其作为血液传播性疾病,应用乙肝表面抗原阳性供体肾脏可能造成乙肝病毒在供受体之间传播,在部分实验中观察到受体乙肝表面抗原转阳等情况,导致受体术后生存率的减少与生活质量的降低。因此需要在不同供受体情况下选择应用相应个体化的预防与治疗手段,减少病毒传播的风险,以及术后随访的过程中通过对相应指标的监测与随访及时发现潜在的病毒传播情况并采取相应对策,期望将不同供受体的移植情况进行系统性总结,为肾移植围手术期治疗方案提供理论与应用参考。

Kidney transplantation has now become the preferred treatment method for patients with end-stage renal disease,but there are still issues with its promotion and development.One of the major factors limiting kidney transplant operations is a lack of donors.Hepatitis B patients'kidneys are valuable source of organ donors.Hepatitis B is a blood-borne illness,and the transplantation of a kidney from a donor who has the virus may spread it from donor to recipient.In some research,the recipient's hepatitis B surface antigen changes to positive that reduced the recipient's life expectancy and quality of life.The risk of virus transmission must therefore be reduced by choosing and putting into practice individualized prevention and treatment measures under diverse donor and recipient conditions.Besides that,potential virus transmission must be promptly discovered through monitoring and following-up on corresponding indicators during the journey of postoperative follow-up,and the necessary countermeasures should be taken.This review aims to methodically summarize the transplantation of various donors and recipients in this review.In order to provide theoretical and practical advice for the perioperative management of kidney transplantation.
