经会阴盆底超声智能识别联合多平面成像技术评估盆膈裂孔形态及功能

Evaluation of morphology and function of pelvic diaphragm hiatus by transperineal intelligent pelvic floor ultrasound combined with multiplanar imaging technology

摘要 目的探讨经会阴盆底超声智能识别联合多平面成像技术在不同分娩方式产妇盆膈裂孔形态及功能评估中的应用价值。方法选取在我院行产后复查的产妇58例,根据不同分娩方式分为经阴道分娩30例(A组)和剖宫产28例(B组),另选30例无流产及生育史的妇女为对照组。各组均应用经会阴盆底超声智能识别联合多平面成像技术测量静息状态和缩肛状态下盆膈裂孔左右径、前后径、面积,以及其对应的相对变化量,并对其进行比较;比较各组肛提肌肌力评分。分析各组盆膈裂孔参数的相对变化量与肛提肌肌力评分的相关性。结果各组静息状态及缩肛状态盆膈裂孔左右径、前后径和面积比较差异均有统计学意义(均P<0.05),各组间两两比较差异均有统计学意义(均P<0.05)。A、B组盆膈裂孔左右径、前后径和面积的相对变化量均小于对照组,差异均有统计学意义(均P<0.05),上述参数在A、B组及对照组中均依次增大,差异均有统计学意义(均P<0.05)。A、B组及对照组肛提肌肌力评分>3分者占比比较差异有统计学意义(P<0.05)。相关性分析显示,盆膈裂孔左右径、前后径和面积的相对变化量与肛提肌肌力评分均呈正相关(r=0.431、0.406、0.532,均P<0.05)。结论经会阴盆底超声智能识别联合多平面成像技术在不同分娩方式产妇盆膈裂孔形态及功能评估中具有较高的应用价值。 Objective To explore the application value of transperineal intelligent pelvic floor ultrasound combined with multiplanar imaging technology in the evaluation of the morphology and function of pelvic diaphragm hiatus in parturients with different delivery modes.Methods A total of 58 cases of women who underwent postpartum reexamination in our hospital were selected,and they were divided into 30 cases of vaginal delivery(group A)and 28 cases of cesarean delivery(group B)according to different delivery modes.Another 30 cases of women without abortion and childbirth history were selected as control group.All groups were underwent transperineal intelligent pelvic floor ultrasound combined with multiplanar imaging technology to measure the transverse diameter,anteroposterior diameter and area of pelvic diaphragm hiatus in the resting state and the anal contraction state,and the relative changes of the transverse diameter,anteroposterior diameter and area of pelvic diaphragm hiatus in the resting state and the anal contraction state.The results were compared.The muscle strength score of levator anal muscle were compared in each group.The correlation between the relative changes of pelvic diaphragm hiatus parameters and the muscle strength score of levator anal muscle were analyzed.Results There were statistically significant differences in the transverse diameters,anteroposterior diameter and area of diaphragm hiatus among the three groups at resting state and anal contraction state(all P<0.05),and there were statistically significant differences in pairwise comparsion(all P<0.05).The relative changes of transverse diameters,anteroposterior diameters and area of pelvic diaphragm hiatus in groups A and B were lower than those in control group,with statistical significance(all P<0.05).The above parameters increased successively in groups A,B and control group,with statistical significance(all P<0.05).The muscle strength score of levator anal muscle in groups A,B and control group were significantly different(P<0.05).The proportion of muscle strength score of levator anal muscle>3 in groups A,B and control group were significantly different(all P<0.05).Correlation analysis showed that the relative changes of transverse diameters,anteroposterior diameters and area of pelvic diaphragm hiatus were positively correlated with the muscle strength score of levator anal muscle(r=0.431,0.406,0.532,all P<0.05).Conclusion Transperineal intelligent pelvic floor ultrasound combined with multiplanar imaging technology has high application value in the evaluation of the morphology and function of pelvic diaphragm hiatus in parturients with different delivery modes.
