摘要
目的探讨术前CT尿路造影(CTU)三维成像对输尿管软镜治疗肾下盏结石效果及安全性的影响。方法将64例肾下盏结石患者根据术前检查方式分为CTU组(n=26)和IVP组(n=38)。CTU组术前采用CTU三维成像,IVP组术前采用静脉肾盂造影(IVP)。比较两组的术前检查指标、手术时间、结石清除率、术后并发症及实验室指标。结果两组的结石直径、IPA、IW比较无统计学差异(P>0.05)。CTU组手术时间短于IVP组,结石清除率高于IVP组(P<0.05);两组的术后并发症发生率比较无统计学差异(P>0.05)。CTU组术后白细胞、CRP水平低于IVP组(P<0.05)。结论术前CTU三维成像对输尿管软镜治疗肾下盏结石的效果显著,可缩短手术时间,提高结石清除率,减少术后炎性反应。
Objective To explore the impact of preoperative CT urography(CTU)three-dimensional imaging on the effect and safety of flexible ureteroscopy in the treatment of lower calyceal calculus.Methods 64 patients with lower calyceal calculus were divided into CTU group(n=26)and IVP group(n=38)according to preoperative examination methods.CTU group received CTU three-dimensional imaging before operation,and IVP group received intravenous pyelography(IVP)before operation.The preoperative examination indicators,operation time,stone clearance rate,postoperative complications and laboratory indicators of the two groups were compared.Results No statistical difference was found in stone diameter,IPA and IW between the two groups(P>0.05).The operation time of CTU group was shorter than that of IVP group,and the stone clearance rate was higher than that of IVP group(P<0.05).No statistical difference was found in the incidence of postoperative complications between the two groups(P>0.05).The leukocyte and CRP levels of CTU group were lower than those of IVP group(P<0.05).Conclusions Preoperative CTU three-dimensional imaging for flexible ureteroscopy in the treatment of lower calyceal calculus has significant effect,which can shorten the operation time,improve the stone clearance rate and reduce the postoperative inflammatory reaction.
