常规肝素盐水封管联合定期尿激酶封管对带涤纶套双腔导管透析患者凝血功能及导管通畅性的影响

Impact of Routine Heparin Saline Sealing Combined with Regular Urokinase Sealing on Coagulation Function and Catheter Patency in Dialysis Patients with Double-Chamber Catheter with Polyester Sleeve

目的探讨常规肝素盐水封管联合定期尿激酶封管对带涤纶套双腔导管透析患者凝血功能以及导管通畅性的影响。方法选取2020年1月至2021年6月于我院带涤纶套双腔导管透析的60例患者,随机分为两组各30例。对照组予以常规肝素盐水封管,观察组在对照组基础上予以定期尿激酶封管,两组均持续干预6个月,比较两组的凝血功能及导管通畅性。结果干预后,两组凝血酶原时间(PT)、凝血活酶(APPT)、D-二聚体(D-D)、纤维蛋白原(FIB)比较差异无统计学意义(P>0.05)。干预后,观察组的导管透析血流量及尿素清除指数高于对照组,静脉压低于对照组(P<0.05)。结论对带涤纶套双腔导管透析患者予以常规肝素盐水封管联合定期尿激酶封管,可增加导管通畅性,但对患者凝血功能影响不大。

Objective To explore the impact of routine heparin saline sealing combined with regular urokinase sealing on coagulation function and catheter patency in dialysis patients with double-chamber catheter with polyester sleeve.Methods 60 dialysis patients with double-chamber catheter with polyester sleeve in our hospital from January 2020 to June 2021 were selected and randomly divided into two groups,with 30 cases in each group.The control group received routine heparin saline sealing,and the observation group received regular urokinase sealing on the basis of the control group.Both groups were continuously intervened for 6 months.The coagulation function and catheter patency were compared between the groups.Results After intervention,no statistical difference was found in the prothrombin time(PT),thromboplastin(APPT),D-dimer(D-D)and fibrinogen(FIB)between the two groups(P>0.05).After intervention,the catheter dialysis blood flow and urea clearance index of the observation group were higher than those of the control group,and the venous pressure was lower than that of the control group(P<0.05).Conclusions Routine heparin saline sealing combined with regular urokinase sealing on coagulation function and catheter patency in dialysis patients with double-chamber catheter with polyester sleeve can improve the catheter patency,but has little effect on the coagulation function of patients.
